- Solicitation No.: 14-245-SOL-00002 (i) This is acombined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional informaiton included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will be issued. (ii) The Request for Proposal (RFP) solication number is RFP: 14-245-SOL-00002. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-66, effective April, 2013. (iv) This requirement is solicited as Full & Open competition. The associated NAICS code is 621399 and the small business size standard is $7 million. (v) See section B.2, page no.3-4, section titled - Fee Schedule/Pricing of the solicitation for a list of contract line item numbers and items, quantities and units of measure, (including option(s)) (vi) Description of requirements for hte items to be acquired: The Contractor shall provide outpatient Ambulatory Surgical Care services to Indian Health Service (IHS), Contract Health Service (CHS) eligible patients on a non-discriminatory basis. In providing health care services, the Contractor assumes the same responsibilities and provides the same consideration, advice, instruction, personal home care guidance and quality of treatment of IHS patients under the terms of this contract as is provided to the general public. The Indian Health Service (IHS) has instituted a payment policy which was published in the Federal Register to contract for health care services under Contract Health Service (CHS) programs for eligible American Indians and Alaskan Natives only with those health care providers who agree to accept, as payment in full, reimbursement at rates no higher than the prevailing Medicare Reimbursement Methodologies including deductible and coinsurance for allowable charges. It is the intent to establish Multiple Award Indefinite-quantity type contracts with those providers who will accept prevailing Medicare allowable rates as reimbursement for authorized health care service rendered. (vii) The period of performance shall be for a 12-month base period with four 1-year options, to commence on the date of award. (vii) The provision at 52.212-1, Instructions to Offerors-Commerical, applies to this acquisition and a statement regarding any addenda to the provision. (ix) The provision at 52.212-2, Evaluation-Commerical Items, apply to this solicitation, and the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) The Contractor's facility shall be The Joint Commission accredited or certified by CMS for the terms of the contract; (2) Professional staff, directly hired by the facility or contracted with the facility shall be licensed in the state in which the facility is located and board eliegible/certified by the appropriate medical/professional board; (3) The Contractor shall possess Medical Insurance Liability in accordance with FAR 52.237-7, Indemnification and Medical Liability Insurance. In addition, the contractor shall provide evidence of general liability insurance for all employees of the facility that provide direct patient care, including x-ray, laboratory and pharmacy; and (4) The Contractor shall have the ablity to bill Alternate Resources in accordance with 42 CFR 136. (x) Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications- Commercial Items, with its offer. (xi) Clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and a statement regarding any addenda to the clause. (xii) Clause at 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commerical Items, applies to this acquisition and a statement regarding which, if any, of the additional FAR clauses cited in the clause are applicable to the acquisition. (xiii) Additional contract requirement(s) or terms and conditions determined by the contracting officer to be necessary for this acquisition and consistent with customary commercial practices are located in Section C - Contract Clauses, of th solicitation. (xiv) The Defense Priorities and Allocations System (DPAS), is not applicable to this solicitation. (xv) This solicitation is issued on an "Open Continuous" basis therfore, responses will be consdered until the official closing date of: February 28, 2015.
