Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The United States Property and Fiscal Office, 9 National Guard Road, Columbia, South Carolina intends to award a firm fixed priced contract for a Mobile Occupational Medical Surveillance Examination Program consisting of providing job-related medical surveillance examinations and occupational health services to South Carolina Army National Guard (SCARNG) full-time support (FTS) and temporary personnel who are potentially exposed to health hazards in the work environment or who are assigned to positions requiring specific standards of physical fitness. Some personnel will receive comprehensive exams; others will receive limited testing. The Contractor shall conduct each test for the price of that test as stated in the schedule (SF1449). Quantities are shown on the schedule as NTE for pricing purposes. The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical professional aspect of services rendered (e.g., treatment, etc). The contractor will provide all labor, tools, supervision and supplies to conduct mobile medical surveillance/occupational health services for the SCARNG as per the attached Performance Work Statement (PWS). Solicitation, W912QG-14-T-0008 is issued as a request for quotes (RFQ) in accordance with FAR Parts 12 & 13. This procurement is 100% small business set aside. The NAICS code for this acquisition is 621111. To be considered for award, the offeror must be registered in System for Award Management (SAM), which can be accessed as follows: https://www.sam.gov/portal/public/SAM/. All vendors MUST have a publicly visible registration in SAM. Offerors must include with their offer, the following information: Duns Number, CAGE Code Number, and Federal Tax ID Number. All Requests for Information (RFIs) (questions) shall be sent to sharon.l.zimmerman8.civ@mail.mil no later than 1000 (10:00 AM ET) Friday, 28 March 2014.
