Description
The Department of Veterans Affairs will be posting a solicitation for "Brand Name or Equal" for an imaging system similar to the Medistim VeriqC as well as various probes. This equipment is specifically designed for the intraoperative blood flow assessment of newly constructed arterial or venous bypass grafts. In addition one of the two devices mentioned above has an attachment that can help locate blood vessels buried in the wall of the heart. There are times when the native heart vessels are not visible to either the surgeon's magnified vision or identified by palpation (felt by fingers) when the heart is manipulated during surgery. When we cannot see or feel the artery then the surgeon must start dividing normal heart muscle in order to expose the artery. This dissection is painstaking and leads to unnecessary heart injury. Being able to find these hidden vessel in a timely manner is essential particularly for the sicker patients where the shorter the operation the better it is for the patient. Not only is total blood flow important (cc's per minute) but also the timing of the blood flow is important. Normally, coronary (native heart arteries) blood flow occurs predominantly during the relaxation phase (diastole) of the cardiac cycle. When the heart is relaxed there is less resistance for the blood to enter the microcirculation. Both of these devices can measure the amount of diastolic blood flow compared to systolic (contraction=squeezing) blood flow. Well-constructed surgical bypass grafts have blood flow patterns that are similar to the native vessels-the majority of the flow SHOULD occur during diastole. These two devices are also capable of demonstrating in graphic form, beat-to-beat, the ratio of diastolic to systolic blood flow. The importance of these measurements are that newly constructed bypass grafts with poor total blood flow and/or grafts with equal (diastole = systole) or systolic-dominant flow patterns may close OFF early. Some bypass graft close early without any change in a patient's condition. Sometimes however early graft closure can be catastrophic. The patient may suffer a new myocardial infarction [=heart attack] and need to return to the catheterization lab for stents or to the OR for an emergent REPEAT coronary bypass operation. We need the ability to assess the surgically constructed bypass grafts in the operating room before closing the chest. This will allow us to make an intelligent, consistent and objective assessment of the quality of the graft. It will be up to the surgeon to decide what to do with the information once it is obtained. The other devices on the list you have asked us about are used for cardiac imaging outside of the operating room. They are used either in the cath lab or in a radiology suite. They are not designed for use in the operating room. This will be posted as unrestricted to the open market.
