- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-39. Submit written quotation on RFQ number W912HP-14-T-0002. The associated North American Industrial Classification System (NAICS) code for this procurement is 532490, Other Commercial and Industrial Machinery and Equipment Rental and Leasing, with a Size Standard of $30 Million. This requirement is being Solicited as a 100% Small Business Set Aside Requirement. The Government intends to award a Firm-Fixed-Price (FFP). The Contractor must be registered in DOD System for Award Management (SAM) prior to award of this solicitation. The solicitation and any subsequent amendments will only be available in electronic format, downloadable from the internet. No CD-ROMs or hardcopy documents will be distributed. Bidders must be registered in SAM and at the Federal Business Opportunity Homepage (www.fbo.gov) in order to view or download the available documents. It is the responsibility of bidders to monitor the website for notices and amendments Instructions, Conditions, and Notices to Offerors CONTENT AND FORMAT OF SUBMISSION: Quotation submittal and all other required documents must be submitted in accordance with this RFQ. Failure to submit the aforementioned information correctly shall make your offer non-responsive, and it shall not receive consideration for award. Please provide a complete price quote and attach a copy of your CAGE Code, DUNS Number, Tax Identification Number, discount terms, and expiration date to your quote. A quotation submitted in response to this solicitation must be submitted electronically or mail and be comprised of the following Sections: SEE ATTACHED REFERENCE DOCUMENTS: - Statement of Work Additional Information: Contractor shall submit invoices on a monthly basis. Payment will be made monthly in arrears. Evaluation Criteria The Government intends to award a contract to the responsible offeror whose quote is the Lowest Price Technically Acceptable quote/offeror after an evaluation is conducted in accordance with the technical factors in this solicitation. The technical evaluation criteria set forth below have been developed by the program office and have been tailored to their requirements in this particular solicitation. Vendor must include descriptive literature such as illustrations, drawings, or a clear reference that demonstrates the furnished item clearly meets the government's specifications. In order to be considered for award, the quote must be determined to be technically acceptable. The technical evaluation factors for this procurement are Lowest Price Technically Acceptable (LPTA). Criteria under the LPTA process are judged on a pass-fail basis only. No other rating or qualitative value will be assigned. To be determined technically acceptable, all factors must be acceptable to obtain a pass rating for the technical quote. Any technical quote that does not receive acceptable assessment for all factors will receive a fail rating for the technical quote. Award will be made to the lowest-priced, technically acceptable, responsible offeror, with satisfactory past performance. The contractor will provide a technical overview of their abilities to perform the services herein. The technical proposal must address the technical factors described herein. Your quote submittal must, at a minimum show: (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) A technical description of the company's items being offered in sufficient detail to evaluate compliance with the evaluation factors in this solicitation; (5) Price and any discount terms; All quotes shall include price(s), FOB point, a point of contact, name and phone number, Also please provide your monthly, weekly, and daily rates. CONTRACTOR RESPONSIBILITY: 1. Equipment delivery and removal at no cost to the government. 2. Provide all parts requiring repair or replacement under circumstances of normal equipment wear and tear at no additional charge 3. The contractor shall dispatch a representative that will arrive onsite in response to a service request to repair and/or replace the chill water unit within four (4) hours of notification by the Government at no additional charge. 4. When the equipment is required to be removed to the contractor's facility for repair or replacement, the contractor shall be responsible for loss or damage from the time it leaves the government site until it is returned to the government.
