Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested. The solicitation number VA248-14-Q-0767 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. The associated North American Industrial Classification System (NAICS) code for this procurement is 337211 with a small business size standard of 500 employees. The Government anticipates awarding a firm fixed price contract. This requirement is a Small Business set-aside. Only Small Vendor quotes submitted will be accepted by the Government. The solicitation will start on the date this solicitation is posted on and will end on 04-03-2014, 15:00:00 Eastern Time. Questions and/or quotation will be emailed to the POC list below. The VHA NCO 08 - 673 - Tampa, FL requires the following items, "Brand Name or Equal", to the following: Furniture List and Specifications Attachments B & C consist of the following Item Numbers: Loewenstein (Manufacturer) #T1: Scope Drum Round Table #9580-A1S - 8 each #T1P: Scope Drum Round Table with Power #9580-A1S (POWER) - 14 each #T12: Scope Drum Cocktail Table #9579-A1S - 2 each #T19P: Scope Drum Square Table with Power #9577-A1S (POWER) - 8 each #T19: Scope Drum Square Table #9577-A1S - 21 each See Attachment D for Statement of Work Required Delivery Date- May 15, 2014 Delivery shall be F.O.B. Destination to the following address: Primary Care Annex 13515 Lake Terrace Lane Tampa, FL 33637 New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Sellers are allowed to submit bids that either meet or exceed the requested specification. Sellers MUST enter exactly what they are bidding (including make, model, and description) for the bid to be considered. Seller whose offer conforms to the solicitation will be most advantageous to the Buyer on the basis of price, technical capability, delivery, and past performance. Bid must be good for 30 calendar days after close of Buy and shipping must be free on board (FOB) destination CONUS (Continental U.S.). The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.212-1, Instructions to Offerors - Commercial Items, 52.212-2, Evaluation-Commercial Items applies to this acquisition; FAR 52.212-3, Offeror Representations and Certifications - Commercial Items - the selected offeror must submit a completed copy of the listed representations and certifications; FAR 52.212-4, Contract Terms and Conditions - Commercial Items; FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, paragraph (a) and the following clauses in paragraph (b): 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34.
