Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number VA249-14-Q-0411 is issued as a request for quotation (RFQ). This requirement is a Small Business set-aside. The associated NAICS code is 811219and the small business size standard is $19 million. The government anticipates awarding a Firm Fixed Price award. Award will be made based on lowest price technical acceptable (LPTA) which conforms to the requirements within this solicitation. Description of Services: Contractor shall furnish all labor, transportation, parts, and expertise necessary to provide repair to the Philips Digital Diagnostic Rad Unit at the Robley Rex VA Medical Center (Serial Number: 08020024, Located in Room A219-1). The contractor shall arrive on station to perform the repair within 24 hours of award. The following issue is to be addressed: System will not do certain exams, stitching, long legs, and standing knee exams. Unit will not track on the wall Bucky. The following parts are believed to be required: 4512-133-01041, Vert. Cont. Firmware and 4512-133-02007, Ver. Cont. Controller PCB. Any additional parts or labor beyond 4 hours must be pre-authorized by the Contracting Officer Representative (COR).The service is required for Louisville VAMC, Engineering (138), 800 Zorn Avenue, Louisville, KY 40206 Period of Performance: 1 day after date of award. Quote Submission: Contractor shall submit their quote on company letterhead and shall include unit price, total, unit qty and item description as specified above, as offered discounts, proposed delivery time, name, address, and telephone number of the offeror, firm's DUNS# and ORCA document in SAM at www.sam.gov, terms of any express warranty, unit price, overall total price, applicable shipping charges, completed copy of 52.212-3 Offerors Representations and Certifications- Commercial Items, and ORCA document. Quotes must be received by Tuesday, April 1, 2014 @ 8:00am. Email your quote to lashanda.harrington@va.gov.
