REQUEST FOR QUOTATION: This requirement is for National Fire Protection Association (NFPA) 101HC and 99 instructor led on-site courses for twenty-five (25) employees at the Bay Pines VA Health Care Facility, 10000 Bay Pines Blvd, Bay Pines, FL 33744 in accordance with the attached Statement of Work (SOW). This is a 100% Small Business Set-aside under NAICS Code 611519 with a $35.5M Size Standard. Note that the current SCA Wage Determination is attached as a reference for ensuring wage rates are at or above the minimum required amount. Delivery Information: The period of performance is 90 calendar days after Contract Award. Classes will be conducted over five (5) consecutive business days (Mon-Fri). The class dates will be determined by the Bay Pines Medical Center Fire Chief with a minimum of three (3) weeks advance notice. Method of payment will be Electronic Payment by Electronic Funds Transfer. Upon completion of work all Invoices from the contractor shall be submitted electronically in accordance with VAAR Clause 852.232-72 Electronic Submission of Payment Requests. Offerors shall submit proposal with RFQ number "VA248-14-Q-0807" noted in subject line of email along with the following: A. Cover Letter: "Cover letter" and introduction shall include the name and address of the contractor submitting the quote, together with the name, title/position, address, email address, fax number, and telephone number of the contact person who has authority to represent the contractor relative to the proposal and any resultant contact. B. Technical Capability: Offerors shall submit course agenda, confirm that a minimum of 3.2 CEUs through an accredited and authorized provider by the International Association for Continuing Education and Training (IACET) will be provided, and evidence of the firm's ability/experience in providing the requirements per the Statement of Work contained within the RFQ. C. Price: Offerors shall complete and submit the 1449 Solicitation (VA248-14-Q-0807) with pricing inserted in CLINS 0001, 0002, and with the Total Cost. Offerors shall be evaluated for fair and reasonableness and not ranked or rated, as this is LPTA. D. Past Performance: Offerors shall provide historical data verifying a minimum of (10) ten NFPA 101 and 99 courses have been taught by a qualified Fire Protection Engineer Instructor on the latest edition of the code. Offerors shall be evaluated as pass/fail and not ranked or rated. Quotes must represent best value and result in the lowest overall cost alternative (including technical acceptable) in accordance with the requirements and delivery. The agency will award a Firm-Fixed Price (FFP) contract using the LOWEST PRICE TECHNICALLY ACCEPTABLE (LPTA) methodology. The Government reserves the right to make award without discussions. To receive an award, contractor must be registered in the System for Award Management (SAM) database (www.sam.gov).
