- Vendor to provide a unique service to the hospital by providing edited copies of movies. The product will be of acceptable content, language and depiction for a representative typical to a hospital audience. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. Quotes are being requested and a written solicitation will not be issued. (ii) RFQ VA250-14-Q-0266 (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. (iv) Full and open competition preference will be given to small business, must prove fair and reseasonable. (v) Contract line item number(s) and items, quantities and units of measure, are as follows; Item 1 - Base Year Movie Channels, IAW SOW _________ Annual Cost Item 2 - Opt. Year 1 Movie Channels, IAW SOW _________ Annual Cost Item 3 - Opt. Year 2 Movie Channels, IAW SOW _________ Annual Cost Item 4 - Opt. Year 3 Movie Channels, IAW SOW _________ Annual Cost Item 5 - Opt. Year 4 Movie Channels, IAW SOW _________ Annual Cost (vi) Description of requirements for the items to be acquired. Statement of Work 3-27-2014 Chillicothe VAMC Movie Channel/DVD's 1. Background: Vendor to provide a unique service to the hospital by providing edited copies of movies. The product will be of acceptable content, language and depiction for a representative typical to a hospital audience. 2. Scope: The programs will be available on one television channel (we program the channel). They will also provide us with DVD copies for the use by Rec Therapists and in our Cinema room. In addition to the movies, the vendor has provided us with health broadcasts that are used between movies to educate our Veteran population on health issues. The modulator will be connected through our CCTV equipment and will be updated every three months by our staff. Staff will change the schedule on a monthly basis in the computer and a schedule will be made and published. The schedule will include an intermission and health issues. The type of movies that will be shown and available shall the latest released entertainment movies that are also available to the general public. The difference with these movies is that all sexual content is edited, as well as profanity. The awardee must have permission from the motion picture industry to edit these movies. Pricing shall also include this license agreement. 3. Tasks: Vendor will provide hospital with product in quantity of 30 digital based movie titles that have been downloaded on the digital media player (hard drive). In addition, they will provide 30 movie titles in DVD format which will be utilized by the Recreational Therapists. The titles will be exchanged every three months. Pricing includes shipping and handling charges of the DVD's and equipment shipments. Vendor certifies it does indeed have exclusive Public Performance rights for hospitals to all motion picture titles it licenses to the hospital. 4.Deliverables: A total of 30 digital based and DVD movie titles delivered every 90 days 5.Place of Performance: VA Medical Center, 17273 State Route 104, Chillicothe, OH 45601 6.Period of Performance: The period of performance will be one year from date of contract award plus 4 - one year option periods. (vii) Date(s) and place(s) of delivery and acceptance and FOB point. VA Medical Center, 17273 State Route 104, Chillicothe, OH 45601 The period of performance will be one year from date of contract award plus 4 - one year option periods. (viii) Provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. 1.Return via email your costs for items 1-5, technical Capability, and past performance. Past performance shall be recent ( within previous 12 months) and relevant ( directly applicable to the requested type of service) for tis request for quote. 2.Deadline for quote submission is 4/9/14 5 p.m. Eastern Standard Time. 52.212-2 -- Evaluation -- Commercial Items. Evaluation -- Commercial Items (Jan 1999) (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: _______price____________________________________ ______technical capability__________________________ _____past performance___________________________ (i) technical capability of the item offered to meet the Government requirement; (ii) price; (iii) past performance (see FAR 15.304); (iv) small disadvantaged business participation; and include them in the relative order of importance of the evaluation factors, such as in descending order of importance.] Technical and past performances, when combined, are as important as price. b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
