- The Bidder's Library is available only to Interested Sources who will be submitting a proposal in response to Solicitation FA5000-13-R-0015 and with an approved DD Form 2345 on file with our office. More information will be added as it is available. To request explicit access to the contents of the Bidder's Library, please accomplish the following: - Access http://www.dlis.dla.mil/jcp/Default.aspx # and click where it says "Online Form" (should be hyperlinked). - Complete the online DD Form 2345 per the instructions, print and mail to: U.S./CANADA JOINT CERTIFICATION OFFICE DLA LOGISTICS INFORMATION SERVICE FEDERAL CENTER, 74 WASHINGTON AVE., NORTH BATTLE CREEK, MI USA 49037-3084 - Once the vendor receives the completed DD Form 2345 from the JCO, email a copy of this form to patrick.hall.4@us.af.mil, Kazzandra.dexter@us.af.mil, lucian.reaves@us.af.mil and anthony.torres.17@us.af.mil. - The Vendor will then need to request access within FBO. That access will be granted within the FBO site upon receipt of their approved DD Form 2345.
