- RFP 14-161-SOL-00011 Past Performance Questionnaire Specifications, Vol 2, Specialty Services Clinic, Whiteriver hosptial Specifications, Vol 1, Specialty Services Clinic, Whiteriver Hosptial Drawings for Specialty Services Clinic, Whiteriver Hospital The Indian Health Service (IHS), Division of Engineering Services-Dallas, is soliciting a small business set-aside for construction of a Specialty Services Clinic (SSC), previously identified in FBO as the "New Whiteriver Dental Clinic", on the campus of the Whiteriver Hospital located on the Fort Apache Indian Reservation in Whiteriver, Arizona. The SSC is a 22,000 square feet single story masonry building, slab on grade and elevated slabs, with steel roof structure. Work includes all finishes and systems, as well as site demolition, utilities, grading and parking. The government anticipates awarding a firm fixed-price contract from this solicitation. The magnitude of this project is between $5,000,000 and $10,000,000 and is 100% set-aside for small business participation. The Small Business Size Standard that applies is $33.5 million average annual revenue for the company's past three fiscal years. Proposals submitted by firms that do not meet this Size Standard will not be considered for award. Offers are solicited only from business concerns that have active registrations in SAM.gov, and North American Industry Classification System (NAICS) Code 236220 must specifically be included in the Offer's approved business plan. FAR Part 15 source selection procedures will be used to select the proposals that represent the best value based on the selection criteria specified in the Request for Proposal (RFP). A Bid Bond for the peal sum equal to 20% of the proposed price must be submitted with proposals. Performance and Payment Bonds will be required from the awardee for a penal sum equal to 100% of the negotiated price will be required as well. A pre-proposal conference and Site Visit for this solicitation will be conducted April 16, 2014; please see Section L-2 of the RFP for further details. Those firms intending to submit a proposal are strongly encouraged to attend the conference. All documents related to this acquisition are attached to this Notice and available to be downloaded at this fbo.gov webpage. No hard copies of the RFP or Attachments will be available, nor will telephone requests be accepted. Offerors are responsible for downloading of all documents pertaining to this solicitation.
