Naval Medical Center San Diego requests responses from qualified sources capable of providing the following items: Room Renovation CLIN 0001: Ultrasound room renovation as per the attached statement of objectives Unit of Issue EA QTY 01 SITE VISITS WILL TAKE PLACE ON APRIL 15TH, 2014 FROM 0930 - 1030 PACIFIC DAYLIGHT TIME. STATEMENT OF OBJECTIVES (SOO) Renovate ROOM 2E-21E5-CD 1.0 Scope The Naval Medical Center San Diego (NMCSD) is seeking offers to renovate the Radiology Dept. Room # 2E-21E5-CD. The renovation will accomplish the objectives listed below to make the room safe and fit for patient use. 2.0 Objectives 2.1 Work shall include replacement of all electrical covers to eliminate open troughs, raceways and boxes; removal and/or grinding flush to the slab all existing anchor studs; replacement of all damaged acoustic ceiling and vinyl flooring tiles; patch and paint where needed throughout the room; discard the beam bar of old/unused x-ray machine; and install rubber vinyl baseboard throughout the perimeter of the room. *(See attached photos)* 2.2 The contractor shall propose a detailed renovation plan, (Statement of Work), which includes the required infection control confinement. The Safety Department and Facilities Management Department must approve the plan before renovation occurs. 2.3 The contractor shall provide the proper mix of qualified personnel for proper repair and renovation. 2.4 The contractor shall provide all necessary supplies, spares, tools, consumables and hardware required to complete this requirement. 2.5 The contractor shall provide a one-year warranty following construction acceptance. 3.0 Site Management 3.1 NMCSD requires the Contractor to provide supervision and project management that will act as a single point of contact to provide the coordination, delivery and integration of all components of this project. 3.2 The contractor shall conduct project planning to ensure that installation will minimally impact daily operations in the designated facility. 4.0 Physical Access Control Navy Commercial Access Control System (NCACS) program implementation instruction (August 2011) 4.1 In accordance with CNICNOTE 5530 dated May 2, 2011, NCACS credentialing should be established in order to avoid unnecessary delay at military installation entry control points (ECP). 4.2 NCACS guidance for Vendors and Contractors to obtain a pass is accessible through the following website: http://www.cnic.navy.mil/regions/cnrsw/installations/navbase_san_diego/om/force_protection/rapid_gate.html 4.3 For more information or to enroll in the NCACS program call: (877) 727-4342. The following factors to be used to evaluate offers: Technical Capability, Defined as ability of the offeror to meet the objectives listed in the SOO. Past Performance; offeror must submit references from similar work performed in the last three years. Price. Technical capability and Past Performance, when combined, are significant evaluation factors that will be most advantageous to the Government. Contractors who do not meet the first 2 factors will not undergo a price evaluation.
