Attachment to SOW Attachment to SOW Terms & Conditions Addendum SF 18 Statement of Work THIS IS A FULL & OPEN COMBINED SYNOPSIS/SOLICITATION UNDER NAICS CODE 517110: The Center for Information Technology (CIT), National Institutes of Health (NIH), is seeking a qualified Business to provide professional services in the further development, integration, and maintenance of Dark Fiber Connectivity to locations in and around the Washington, DC Metropolitan Area; Baltimore City Metropolitan Area; and other selected locations as required to support the NIH and other Operating Divisions (OPDIVS) of the Department of Health and Human Services (HHS). Work shall be performed in accordance with the requirements specified in the attached Statement of Work (SOW). This is a combined synopsis/solicitation for Commercial Items prepared in accordance with the format at Federal Acquisition Regulation (FAR) Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items; and FAR Subpart 13.5, Test Program for Certain Commercial Items, as supplemented with additional information included in this announcement. This solicitation incorporates provisions and clauses in effect in the March 2005 FAR Revision, including all FAR Circulars issued as of the date of this synopsis. The following clauses and provisions cited herein are incorporated by reference into this solicitation and may be obtained from the web site https://acquisition.gov/far/current/html/52_212_213.html: FAR 52.212-1, Instructions to Offerors - Commercial (July 2013), and FAR 52.212-4, Contract Terms and Conditions - Commercial Items (September 2013). FAR 52.212-2, Evaluation - Commercial Items (January 1999), also applies with the following two evaluation criteria (each having equal value) to be included in paragraph (a) of the provision: (1) Corporate capabilities and past performance in providing professional services in developing, integrating, and maintaining Dark Fiber Connectivity for projects of the magnitude specified in the attached SOW, including a response to the NIH Information and Physical Access Security Requirements specified in Provision 10.0 of the attached SOW; and (2) Experience and expertise of proposed staff with Dark Fiber Connectivity projects of the magnitude specified in the attached SOW. An award will be made to the offeror who represents the best value to the Government whereby the two (2) evaluation criteria will receive paramount consideration in the selection of a contractor. However, cost/price may become a critical factor in source selection in the event that two or more offerors are determined to be essentially equal following the evaluation of all factors other than cost or price. It is anticipated that any order awarded from this announcement will contain option provision(s) and period(s). In accordance with FAR Clause 52.217-5, Evaluation of Options (July 1990), the Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement, except when it is determined in accordance with FAR 17.206(b) not to be in the Government's best interests. Evaluation of options will not obligate the Government to exercise the option(s). Sources having the experience and expertise to provide the Dark Fiber Connectivity requirements specified in the attached SOW shall submit clear and comprehensive information supporting their proposal. An offeror must submit information in sufficient detail to allow the Government to evaluate the offeror's previous experience and effectiveness in similar or closely related work, its current ability to accomplish the work requirements specified in the attached SOW, and to allow the Government to perform a basic analysis of the proposed prices. In any event, the Government reserves the right to make an award to that offeror whose proposal provides the best overall value to the Government. Offerors must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (November 2013), with their offers. The clause at FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (January 2014), and the attached Addendum to Terms and Conditions of Purchase Order apply to this acquisition, as well as the following FAR clauses cited therein: FAR 52.219-6, Notice of Total Small Business Set-Aside (November 2011), and FAR 52.232-33, Payment by Electronic Funds Transfer - System of Award Management (July 2013).
