?.## Scope of Work Repair, Replace, & Re-pipe Steam Traps Site Visit: Thursday April 17, 2014 @ 10AM, Meet in Building 80 Room 110 Bedford VA Hospital 200 Springs Road Bedford, MA 01730-1114 Contractor shall supply all labor, parts, supplies, tools and any specialized equipment needed to repair, replace, & re-pipe steam traps identified as requiring repair, replacement, & re-piping in (Appendix 1), from the Steam Trap Survey conducted in the fall of 2013. The work will be performed between June 1, 2014 and September 1, 2014. Once all work is completed, the Contractor will provide an updated Appendix 2, providing the same criteria as in Appendix 1, 2 hard copies and 1 Electronic copy. In addition to the traps to be repaired, replaced, or re-piped in (Appendix 1), the Contractor supply all labor, materials, tools and specialized equipment to correct items 1-5 below. 1.There are live steam leaks at trap stations 96729, 111203, 111814, and 111234. These leaks will be repaired by the Contractor. 2.Trap number 111892 is not installed level; this trap will be piped correctly by the Contractor. 3.The check valve at trap station 96500 is installed backwards and will be re-piped properly by the Contractor. 4.Trap number 96383 is installed upside down; this trap will be re-piped correctly by the Contractor. 5.Forty five (45) orifice traps were found throughout the facility during the survey, no orifice traps are to be repaired or replaced in this project, these traps will be replaced with properly sized mechanical or thermostatic by the Contractor. Contractor will be working in various types of areas, including but not limited to mechanical, office, and patient settings, and will be cognizant and respectful of the people in and around the area in he is working in. Contractor shall provide a schedule of work to be completed for approval by the Contracting Officers Representative (COR), 4 weeks prior to the start of the work. Contractor shall use direct manufacturer repair or replacement parts. Contractor is responsible for the removal and re-installation of radiator covers and other minor items that may be blocking access to the traps, and for cleaning up his work area before moving on to the next area of repair. Once the work is completed, the heating system will be placed back on line long enough for the contractor to check for leaks and check for proper operation of the repaired, replaced, or re-piped traps. The Contractor will schedule this with the COR. Contractor is responsible for the proper disposal of all materials from the repaired, replaced and re-piped trap assemblies. Contractor will sign in & out daily at building #22 Boiler Plant, and shall attain Contractor Badges at the Main Engineering Office. Badges are to be worn at all times when on the facility, the Contractor will supply a list of the workers names and a copy of their ID, and have the Security Statements completed two weeks prior to the start of the project. This work is being conducted as part of an Energy Efficiency Rebate Program with National Grid of Massachusetts. All available rebates under the program shall be paid directly back to the customer. The contractor is responsible for providing all required documentation, post inspections, and any other contractor requirements as required by the program with National Grid. The COR will provide National Grid Program contact information upon request. There shall be no changes to this Scope of Work without prior approval of the Contracting Officer.
