- Attachment E - U.S. DOL Wage Determinations Attachment D - Security Form Attachment C - Past Performance Questionnaire Attchment B - Bid Schedules Attachment A - Statement of Work DE-SOL-0006643 with Clauses Western Area Power Administration (Western) intends to award one contract for Centralized Helicopter Maintenance Services. This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, which is issued as a Request for Quotation (RFQ); quotes are being requested and a written solicitation will not be issued. Solicitation DE-SOL-0006643 is issued as a Request for Quotation. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. An Indefinite-Delivery contract will be awarded using Simplified Acquisition Procedures. This solicitation is 100% reserved for small business. The North American Industry Classification System (NAICS) is 488190 and the size standard is $30.0 million. Contractor shall furnish all labor, supervision, facilities, materials, equipment, tools and supplies necessary to provide Centralized Helicopter Maintenance Services to accomplish scheduled major inspections, overhauls, major alterations, major repairs, unscheduled major inspections, scheduled 100-hour and annual inspections on 3 Western helicopters, and their components. Work includes time between overhaul (TBO) inspections and replacement of retirement life items listed under the following type certificates: H2SW and E1GL, with latest revisions. Western operates the following aircraft: one Bell Helicopter Model 407, Registration #N617DE, Serial #53867 (based at Loveland, CO); one Bell Helicopter Model 407 N619DE, Serial #53410 (based at Phoenix, AZ); and one Bell Helicopter Model 407, Serial #53798 (based at Bismarck, ND). Attachment A provides the Statement of Work. See Attachment B, Bid Schedule, for the list of Line Items. Offerors shall submit one completed bid schedule to be used for all helicopters. In accordance with the Service Contract Act, the applicable U.S. Department of Labor Wage Determination will apply for the contractor's service facility location. At this time the place(s) of performance is located at Loveland, Colorado; Phoenix, Arizona; and Bismarck, North Dakota. The Contracting Officer will obtain wage determinations for specific places of performance if asked to do so in writing (via email) by April 23, 2014. Term of contract is one base year and four option years. Separate task orders will be issued for individual requirements. The minimum quantity of services is $36,000. If ordered, the contractor must furnish any additional services ordered, not to exceed the stated maximum. The maximum quantity of services is $6.5 million. Attached to this Notice are the provisions and clauses applicable to this acquisition, including the instructions to offerors and the evaluation criteria. As part of the criteria, Pass/Fail Factors include the Offeror is a FAA Certified Repair Station and the Offeror is an Authorized Bell Service Center. Quotes may be submitted by regular or express/overnight mail. Quotes submitted by fax will not be accepted. Quotes will be evaluated based upon the criteria stated in the attached provision at FAR 52.212-2, Evaluation Commercial Items. Contractors must be registered in System for Award Management (SAM) located at website: www.sam.gov to be eligible to receive an award from any Federal agency. Contractors must also complete the On-line Representations and Certifications Application (ORCA) electronically located in SAM; and update its reps and certs annually. Proposals are due by 2:00 PM Mountain Time on May 2, 2014.
