Sources Sought: This is a sources sought announcement only. This is not a solicitation announcement. The purpose of this announcement is to gain knowledge of current market capabilities in relation to NAICS Code 62111 Offices of Physicians (except Mental Health Specialists), Size Standard $10.0 Million and/or NAICS 561320 Temporary Staffing Services, Size Standard $25.5M.. The Network Contracting Organization at Canandaigua (VISN 2) has a requirement for a contractor to provide MOD (Inpatient) and Urgent Care (UCC) physician services during administrative and non-administrative work hours at the Bath VA Medical Center, Bath, New York. Responses to this sources sought must include the following information. 1. General information of your company name, address, contact person name, phone number, fax number, email address(s) and DUNS number and company website if available. 2. Type of business such as 8(a), Small Business, Hub Zone Small Business, Small Disadvantaged Business, Service Disabled Veteran Owned Small Business or Veteran Owned Small Business. 3. Company experience providing medical staff to a VA Hospital. Responses to this market survey should be in writing and shall be e-mailed to the attention of Jeanette Crooks, Contract Specialist at Jeanette.Crooks@va.gov. No telephone responses will be accepted.
