SCOPE OF WORK ID#: LBJC2014-001 CENTER PHYSICIAN SERVICES Odle Management Group is honored to be a contractor for the Job Corps program, a national residential training and employment program administered by the Department of Labor. Since its inception, the Job Corps program has successfully addressed the multiple barriers to employment faced by disadvantaged youth throughout the United States. Originally established by the Economic Opportunity Act of 1964; current authorization for the program is Title I, Subtitle C, of the Workforce Investment Act of 1998, Job Corps provides a comprehensive career development services to students including academic, vocational, social and independent living skills, career readiness training and support services. The unique combination of services provided in the Job Corps is intended to prepare youth to obtain and hold gainful employment, pursue further education or training, or satisfy entrance requirements for careers in the Armed Forces. SCOPE OF SERVICES : 1. Assessment and diagnosis, to include a cursory inspection within 48 hours by a physician or assistant for conditions needing immediate care. 2. Entrance medical examination by the physician or designee within 14 days after entry. 3. Routine laboratory work that can be done on center, e.g. hemoglobin or hematocrit, urine, sugar and albumin, pregnancy. 4. Immunizations as indicated. 5. Tuberculin skin text (Mantoux). 6. Vision and hearing screening. 7. Daily walk-in clinic and appointment system for above and for episodic illness or injury assessment by center physician and/or assistant. 8. Inpatient and (during office hours) for minor conditions, such as upper respiratory infections, or flu symptoms. 9. Primary emergency care for illness and injury, including first aid and CPR, and secondary care within capabilities, e.g., injections of ephinephrine, and immediate transfer to hospital emergency room for specialized diagnosis and treatment, if needed. 10. Treatment of urgent and other conditions not needing specialized care, within capabilities of center physician and assistant, such as contusions and abrasions. 11. Referral to off-center physicians for detailed specialized assessment regarding on-center management of asthma, uncomplicated diabetes, normal pregnancy, etc. 12. When students need prescriptions not provided in the basic formulary, local pharmacies will be given insurance information whenever possible. 13. Waiver of medical care – a) if a student refuses pelvic exam and/or HIV testing, the center may defer it until after specialized counseling, a disciplinary separation may be necessary. The center physician may waive the pelvic examination if in his or her opinion there is sufficient justification and document as such, b) the center physician may grant waivers of immunization requirements for valid medical and/or religious reasons and documented as such. 14. The Center Director shall meet monthly with the center physician to discuss clinical and organizational issues. 15. Collecting date and preparing reports as required by the Center Director and/or the Department of Labor. 16. Cooperating with corporate and Regional Office center assessments. 17. Staying current on Job Corps policies and procedures via frequent communication with the center’s health and wellness manager. The Center Physician agrees to adhere to the policies, procedures, and guidelines established by Job Corps. The undersigned further agree that the Center Physician will have primary responsibility to implement the program outlined above and may be assisted by the Job Corps National Office and/or health consultants. The Center Physician, with the concurrence of the Center Director, may delegate by written and dated health care guidelines any of his/her responsibilities set forth in this contract to appropriate qualified staff. The Center Physician may delegate professional responsibilities set forth in this contract to another full- or part-time physician only with the Center Director's prior concurrence and a prior review of the secondary physician's qualifications by the regional director and regional medical consultant. It is understood that the medical services provided by the Center Physician are subject to monitoring and review by Job Corps. The Center Physician may contact the regional medical consultant with any questions/concerns. OBLIGATIONS OF THE PROFESSIONAL In the performance of the above duties and responsibilities, the Center Physician: A. Agrees to maintain malpractice insurance to cover the professional services to students, with limits consistent with the medical service industry, but in no case less than $1 million per occurrence. B. Agrees to make no public statements regarding its activities under this Subcontract, or to disclose any information regarding Job Corps students or Job Corps activities without prior written approval of the Center Director or his/her designated representative. Unauthorized release of student records is a federal crime punishable by fine and/or imprisonment. C. Shall be responsible for finding and arranging for a professionally qualified replacement/substitute during temporary periods of absence of more than one calendar week. D. Shall be personally and financially liable for the cost of any patient referral outside the Center without prior written approval of the Center Director. E. Certifies its intent to comply with all equal employment opportunity practices and affirmative action policies as required by law and agrees not to discriminate against any student, employee and/or contractor because of race, religion, color, gender, age or national origin. F. Shall provide current copies of all professional licenses, certifications, professional and auto liability insurance documents required by the Center. G. Agrees that all research projects and/or publications relating to Job Corps health activities will first be cleared through the National Office of Job Corps, Washington, D.C. Vendor must include the Scope of Work Identification number on submitted bid. The period of performance is from May 15, 2014 to April 30, 2015 with 3 one year option periods. Submitted bid will show at minimum breakdown as follows: Price per hour Please include any additional fees Offerer must quote on all periods, the base period is 52 weeks and each option is 48 weeks Bids and all licensures needed to do business may be faxed to 562-491.5198, Attn. If unable to quote, please indicate 'No Bid' and return bid package to us. Odle Management Group, Inc. reserves the right to reject any bids and select the bid deemed most responsible. Bidders should be aware of bid requirements for federally funded projects and act accordingly. Bidders are placed on notice that this is a competitive bid process and cost is an important fact. Overall low bid does not constitute bid award. Long Beach Job Corps Center is a federally funded facility. Payment terms are Net 30 days from date of invoice. Vendor is required to show number of hours billed and hourly rate on invoice, list of materials used to complete service and provide certified payroll records with invoice. All question regarding the Invitation to Bid and/or scope of work must be submitted in writing to: Young.Peter@jobcorps.org. No phone calls will be accepted Vendor shall hold all licenses necessary to do business in the state of CA and provide copies of all licenses with bid package. The vendor shall obtain and maintain insurance as follows: • Workers Compensation insurance to cover obligations imposed by federal and state statutes having jurisdiction of its employees engaged in the performance of the services represented by this contract, and Employers’ Liability insurance with a minimum limit of $500,000. Evidence of qualified self-insured status shall also suffice for this section. • Professional Medical Liability insurance with a minimum combined single limit of $1 million for each occurrence and an aggregate of $3 million. • General Liability insurance with a minimum combined single limit of $1 million per occurrence and in the aggregate (or letter, per below). • Where appropriate, Comprehensive Business Automobile Liability insurance with a combined single limit of $1 million per occurrence and in the aggregate. • The proposed shall cover any and all expenses not covered by said insurance including all deductibles. Bids will be due by Sunday May 11, 2014 at 5:00 East Coast Time
