- Amendment #1 The purpose of this amendment is to provide for an extension to the due date for submission of responses until Monday, April 21, 2014 at 12:00 p.m. ET and to notify interested parties that there was an error in the login information provided to companies that requested access to Appendix 15. The Contracting Officer will be sending corrected login information to those that have requested it. In the event that you do not receive the corrected login information, please contact the points of contact listed on this notice. All requests for access to Appendix 15 shall be submitted in writing via email. Phone call requests will not be accepted. Thank you for your interest. ======================================================== Notice to Small Businesses The purpose of this Small Business Sources Sought Notice is to seek declarations of demonstrated technical capabilities and various information from qualified small business concerns [Woman-owned Small Businesses (WOSB), Historically Underutilized Business Zone (HUBZone) Small Businesses, Service-Disabled Veteran-owned Small Businesses (SDVOSB), and 8(a) Small Businesses]. The U.S. Food and Drug Administration (FDA) intends to provide maximum practicable opportunities in its acquisitions to small business. Background This Small Business Sources Sought is for information, planning and market research purposes only and shall not be construed as either a solicitation or obligation on the part of the FDA. FDA will use this market research information to assess the market's capability to successfully meet FDA's IT Helpdesk requirements. FDA welcomes responses from all interested parties. FDA does not intend to make a selection decision or award a contract on the basis of responses, nor otherwise pay for the preparation of any information submitted or FDA's use of such information. Acknowledgment of receipt of responses will not be made, nor will respondents be notified of the outcome of the FDA's review of the information received. Additionally, the FDA does not intend to hold discussions concerning this Small Business Sources Sought with any interested parties. However, FDA reserves the right to contact vendors if additional information is required. General Instructions Responses to the Small Business Sources Sought should not exceed 20 pages in length. A complete response to the Small Business Sources Sought shall include answers to all of the questions below. Please note that a respondent's failure to provide a complete response to a question will be interpreted to mean that it does not understand the question and does not demonstrate the capability to provide services required. Responses shall demonstrate capability, not merely affirm the respondent's capability (e.g.: The response must go beyond the statement that, "XYZ company can provide helpdesk support."). Responses shall be submitted via email to the primary and secondary points of contact listed above, by the date and time specified in this notice. Questions 1. Provide a detailed description of work you have performed within the past three years, with Government agencies and/or private industry, that is of similar size, scope, and complexity to the tasks specified in the attached draft SOWs. The description should emphasize how the quality of work was measured, including compliance with performance schedules and SLAs. 2. Please provide a narrative of at least two instances where you have tracked (e.g. customer surveys, metrics...etc) and met customer satisfaction goals. Please describe your role with respect to tracking and meeting the customer satisfaction goals. Provide details as to the methods used and frequency at which performance against customer satisfaction goals were tracked and reported on. 3. Please provide a narrative of instances where you have effectively managed staff and technical user support requests across multiple disperse geographic areas. Please provide specific information about the contract and your role (prime vs/ sub). If the role was a sub-contractor, please provide your area(s) of responsibility on the contract. 4. Please provide a narrative of instances where you have effectively managed support requests surges due to increased customer demand while continuing to comply with performance schedules and SLAs. Please provide specific information about the contract and your role (prime vs/ sub). If the role was a sub-contractor, please provide your area(s) of responsibility on the contract. 5. What technical capabilities and expertise are resident in the contractor's organization, and what percentage of the work does this represent? What percentage of the technical capabilities and expertise will require the aid of a subcontractor? Please demonstrate your firm's capability to meet the requirements of FAR 52.219-14, Limitations on Subcontracting. 6. Please provide documentation of the size of your business and your classification as a small business, Hubzone small business, SDVOSB, WOSB and/or SBA 8(a) certified small business. 7. Please suggest an appropriate North American Industry Classification System (NAICS) code and provide your rationale. 8. Please provide the following: a. Organization name b. DUNS number c. Organization's website d. Contact Name e. Contact Telephone f. Contact E-mail address g. Size designation h. Number of employees in your organization i. Your organization's current and gross revenue 9. Provide information concerning contract vehicles available to be used by the FDA in acquiring the services outlined in the draft SOWs (GSA Federal Schedules, Government wide Acquisition Contract, etc.).
