Description

REQUEST FOR QUOTE (RFQ) AND MANDATORY SITE VISIT ANNOUNCENENT FOR SERVICE BASED CONTRACT FOR THE INSTALLATION OF APPROXIMATELY 20,000 FT2 OF 4 INCH SLOPE STABILITY SYSTEM SUCH AS PRESTO SUCH AS 4 INCH GEOWEB OR EQUAL IN THE SWALE ALONG THE SOUTH PROPERTY LINE OF THE ORLANDO VA MEDICAL CENTER LAKE BALDWIN CAMPUS. The Department of Veterans Affairs Orlando VA Medical Center has a requirement for a service based contract that will install approximately 20,000 ft2 of 4 inch slope stability system in the swale along the south property line of the Lake Baldwin campus. The existing swale has been damaged due to vehicles parking and getting stuck in the sandy soil. Work will require grading the soil, restoring the swale to a proper slope, installation of the slope stability system, and backfilling. The attached RFQ (Request for Quotation) which includes: RFQ, Statement of Work, Drawings and Wage Determination. The OVAMC anticipates the award of a Firm-Fixed-Price type purchase order. Only those that attend the site visit will be considered for award. The mandatory site visit is scheduled for Thursday April 17, 2014 at 10:00 A.M. EST. Please advise the Contract Specialist, Lisa John, via email at Lisa.John@va.gov of your intent to attend no later than Wednesday April 16, 2014 at 10:00 A.M. EST. If no responses are received from vendors concerning the site visit by this date, the site visit will be cancelled with no further notification. This requirement is 100% set aside for competition among Service Disabled Veteran Owned Small Business. Your company must be registered with a Dun and Bradstreet Number (DUNS) as a small business in the System for Award Management website (www.sam.gov) for consideration for award. This RFQ is not to be construed as a contract or a commitment of any kind. The Department of Veterans Affairs will not pay for any information received or costs incurred in preparing the response to this Request for Quote. Therefore any cost associated with your submission is solely at the interested vendor's expense. All questions pertaining to this announcement should be addressed in writing to the Contract Specialist, Lisa.John@va.gov, at e-mail address Lisa.John@va.gov. All quotes should be submitted electronically by Wednesday April 23, 2014 by 3:00 P.M. EST to Lisa.John@va.gov.
