- Contractors must use this RFQ Quote Sheet to submit pricing. COMBINED SYNOPSIS/ SOLICITATION FOR COMMERCIAL ITEMS (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation/synopsis reference number is F1M3F14086A001 and is being issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72, effective 30 Jan 2014. (iv) This acquisition is 100% set-aside for small businesses. The associated NAICS code is 811111 with a $7 million size standard. (v) Contractors shall submit a quote using the RFQ sheet provided with this announcement. The RFQ sheet is attached to this combination solicitation/synopsis and will be used to submit quotes. After award, this document will become part of the contract. A SITE VISIT will be conducted at 10:00 AM (ET) on Friday, 18 April 2014. All prospective Offerors shall meet at the Joint Base Charleston - Air Base visitor's center located off of Dorchester Road no later than 9:50 AM on the date of the site visit. All prospective Offerors are urged to attend this conference and are requested to contact Terry Harrelson, Contract Specialist at (843) 963-5182, Fax number (843) 963-5183, or e-mail address terry.harrelson.1@us.af.mil No Later Than 16 April 2014 if planning to attend. Offerors should allow sufficient time to obtain a visitor pass (requiring valid driver's license, vehicle registration, and current proof of vehicle insurance.) All responsible sources may submit a quote/proposal, which shall be considered. A firm fixed price purchase order is contemplated. (vi) F1M3F14086A001: Item 0001 - Repair/Replace Diesel Fuel System, 2008 Ford F-550 Van: Contractor shall provide all parts, labor, transportation, supervision, and management in order to inspect/repair/replace all parts necessary, tow the vehicle to the contractors repair shop, and restore the vehicle fuel system to manufacturer's specifications. Items to be repaired/replaced include: pumps, filters, gaskets, sensors, housings, fuel lines, oil, nuts & bolts, and inspection ONLY of the turbo assembly. This list is not all inclusive and does not relieve the contractor from repair/replacement of parts or equipment not listed, and if required Item 0002 - Turbo Repair : Contractor shall provide all parts, labor, supervision, and management in order repair/replace all parts necessary to restore the vehicle turbo assembly identified in item 0001 to manufacturer's specifications. Item 0002 is provided for planning purposes and will be evaluated as part of the overall award. (vii) Repairs must begin within 7 calendar days of award and be completed within performance period identified in the contractors submitted RFQ." FOB Destination - Contractor shall tow the vehicle to their place of business and return to Joint Base Charleston - Air Base. (ix) FAR 52.212-2, Evaluation - Commercial Items Evaluation. The Government will award a purchase order resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The Government will evaluate the total price of the offer for award purposes. (x) All offerors shall include a completed copy of FAR 52.212-3, Offeror Representation and Certifications -- Commercial Item or complete electronic representations and certifications at https//www.sam.gov. (xi) The clause at FAR 52.212-4, Contract Terms and Condition-Commercial Items, applies to this acquisition. (xii) The clause at FAR 52.212-5 CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS COMMERCIAL ITEMS (DEVIATION 2013-O0019) (Jan 2014) (a) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (a) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. (2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. (b) (1) Notwithstanding the requirements of any other clause in this contract, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (b)(1) in a subcontract for commercial items. Unless otherwise indicated below, the extent of the flow down shall be as required by the clause- (i) 52.203-13, Contractor Code of Business Ethics and Conduct (Apr 2010) (Pub. L. 110-252, Title VI, Chapter 1 (41 U.S.C. 251 note)). When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the interested party to another official who can resolve the concern.
