Description

The Robley Rex VA Medical facility in Louisville, Kentucky has a requirement for a contractor to provide all services, planning, labor, equipment, materials and professional supervision to install a modular facility not to exceed 1000 square feet on the medical center campus located at 800 Zorn Avenue. The contractor shall provide a modular facility that is a complete and usable system ready for operations at the conclusion of the project. The modular building shall be less than 1000 square feet, have four (4) Tele Health rooms of approximately 60 square feet each, one (1) class room, two (2) uni-sex restrooms, one (1) housekeeping closet, (one) electric closet and one (1) communications closet. The classroom shall have a space for a staff break area with space for a refrigerator and microwave. Interior wall construction shall use sound proofing construction to restrict sound transference between rooms. The building shall contain a minimum of two (2) entrance/exit doors. The modular facility exterior shall complement existing facility structures in style and exterior finishes. The modular facility shall be compliant with the Department of Veterans Affairs Barrier Free accessibility. The work shall include all building systems and controls necessary for appropriate use. The work shall include all finishes appropriate to a health care environment. Some preliminary planning in establishing the final footprint and floorplan for the work required, and as such the work will include the preparation of complete drawings in AutoCAD 2012 or other format upon agreement with the Robley Rex VA Medical Center. Specifications are to be in 2004 Master Construction Specification Format per the US Department of Veterans Affairs, Office of Construction & Facilities Management (DVA-CFM) guidelines. The work needs to include all disciplines necessary for the completion of the project in accordance with the US Department of Veterans Affairs, Office of Construction & Facilities Management (DVA-CFM) guidelines and specifications (available via website: http://www.cfm.va.gov/til/spec.asp). The work shall conform to current DVA-CFM standards and provide adequate facilities for employees. The floor plan shall incorporate the principles of sustainable design and energy efficiency per DVA-CFM guidelines. The floor plan shall also incorporate Plane Tree patient centric design principles. At final submission, deliver the original Construction Documents to the Contracting Officer Technical Representative after all previous review comments are incorporated and the Construction Documents have been approved. The Construction Document drawings shall bear the seal of the Registered Architect and Professional Engineer responsible for the design. The performance period for this project is 180 Calendar days from receipt of Notice to Proceed. 60 Days will be allocated for the Planning Design Period and 120 Days for the Construction Period services, for a total of 180 Calendar Days. NAICS code 238990 is applicable to this requirement. The small business size standard is $14M. The construction cost range is between $250,000.00 and $500,000.00.
