Description

SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 is conducting a market survey to identify potential sources to provide Biological Tissue Mesh. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and participants will not be compensated. The purpose of this market research is to identify capable sources of providing Biological Tissue Mesh. The mesh is used infrequently, however, it must be available for emergencies and abdominal wall repairs. The mesh is monitored and recorded in a Universal Devices and Implants (UDI) Tracker for storage and usage. In addition, this mesh has an expiration date that it must be used by. As such, the VA is looking to keep smaller amounts on hand. However, the source must be replenished as it is utilized so that an adequate amount if available for patient care. The North American Industry Classification System Code (NAICS) is 339113 and the small business size standard is 500 employees. Capabilities Statement and Documentation: Firms/Companies are invited to provide their capabilities, experience and knowledge in the provision of Biological Tissue Mesh. Sources are asked to provide information to demonstrate their answers to the following items of information: 1.Company Name, Address, Website, POC, and POC Phone Number. 2.The North American Industry Classification System Code (NAICS) is 339113 and the small business size standard is 500 employees for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years. 3.A summary of firm's ability to provide the VA with an adequate supply of mesh on hand as indicated by expiration and usage levels. 4.A summary of the firm's ability to maintain the mesh's quality during shipment. 5.The type of mesh the firm provides and supporting information regarding its quality. In addition to a Capability Statement which provides the above, submissions can include specifications, brochures, and manuals as attachments.
