Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is VA250-14-Q-0301 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. This is a small business set-aside and the associated NAICS code is 444220 and small business size standard is $10 million. The requirement is for contractor to provide and maintain rented silk plants. Weekly services are expected to accommodate infection control requirements. Heights of plants vary. A basic black container is adequate. See attached statement of work for description and requirements. Delivery location is Columbus VA Ambulatory Care Center, located at 420 North James Road, Columbus OH 43219-1834 and shall be FOB destination. Delivery and installation required within 30 days of contract award. Award will be for base with two option periods available. This announcement constitutes the only solicitation; a formal, written solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. This solicitation documents and incorporates provisions and clauses in effect through FAC 2005-72. It is the responsibility of the contractor to be familiar with the applicable clauses and provisions. In addition to filling out the price schedule, offers should include best expected delivery date. ** When combined, Technical and Past Performance are equal to price.** EVALUATION PROCEDURES: The Government will select the best overall offer based on price and the offeror ability to meet the requirements specified in this solicitation. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, will be the most advantageous to the Government in accordance with the requirement. Only those offerors determined to be in compliance with the requirements as stated in this solicitation will be considered for award. Non-conformance with this requirement may result in offeror's quote being determined nonresponsive.
