Description

The Atlanta VAMC is seeking contracts with community hospice agencies to deliver all levels of hospice care to veterans who will receive their care in the 50 county catchment area identified in the solicitation. In order for hospice agencies to be considered for contract, every hospice agency must be: 1) Medicare Certified 2) Licensed to operate a hospice agency in the state of Georgia. 3) Hospice agencies must also have and maintain accreditation with a recognized hospice accrediting agency such as Joint Commission or CHAPS. With this large geographic area, it is expected that multiple contracts will be awarded from this solicitation. In the event there are multiple requests for contracts for the same service area, the following criteria will be taken into consideration to award the contract: 1) Own and operate a free standing hospice inpatient unit. 2) Hospice Medical Directors are certified Hospice and Palliative care physicians. 3)Availability of supportive therapies or programs to enhance care beyond MCR requirements such as; music therapy, pet therapy, Living Legacy programs, massage therapy, etc ¦ 4) Same Day admissions (i.e. after hours). 5)Single referral center and point of contact if agencies operate multiple branch offices. 6) Greater than 50% of nursing staff are hospice and palliative care certified. 7) Greater than 30% of home health aides are hospice and palliative care certified. 8) Quality improvement program in line with QAPI guidelines with measurements/outcomes scoring greater than 60% or serial reports reflecting at least a 5% improvement over time.
