This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested. The solicitation number VA248-14-Q-0881 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510 with a small business size standard of 500 employees. The Government anticipates awarding a firm fixed price contract. Responses must be concise and be specifically directed to the requirement referenced below. The solicitation will start on the date this solicitation is posted on and will end on 04-24-2014, 15:00:00 Eastern Time. Questions and/or quotation will be emailed to the POC list below. The VHA NCO 08 - 673 - Tampa, FL requires the following items, EXACT MATCH, to the following: LI 001: Artic Sun 5000 Temperature Management System, Cat# 50000000, 1EA LI 002: Artic Sun 5000 Calibration Test Unit (CTU), Cat# 741-00, 1EA LI 003: One Year Standard Warranty LI 004: Artic Sun 5000 Temperature Management System Cover, Cat# 752-00, 1 EA LI 005: Artic Sun 5000 Service Kit, Cat#771-00, 1 EA LI 006: On-Site Staff Training LI 007: 24/7 Clinical/Technical Support LI 008: One Year Extended Warranty Required Delivery Date- May 30, 2014 Delivery shall be F.O.B. Destination to the following address: James A. Haley VAMC 13000 Bruce B. Downs Blvd Tampa, FL 33612 New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Exact Match Only The Buyer requires that Sellers bid 'exact match to spec'. That means that NO SUBSTITUTIONS are allowed. Sellers MUST provide the item requested in the line item description (exact make, model, part number and or description).
