- Solicitation #: HSBP1014Q0064 Pricing Template Attachment B, Statement of Work Attachment A. CBP Directive No. 51410-023 Physical Fitness Program's (Pilot) • I. I. COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS : This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. • II. II. REQUEST FOR QUOTES (RFQ) : This is RFQ #: HSBP1014Q0064. • III. III. FEDERAL ACQUISITION CIRCULAR : This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-72 effective January 30, 2014. • IV. IV. NAICS CODE AND NO SMALL BUSINESS SET-ASIDE : The NAICS code for this solicitation is 713940 and the Small Business set-aside standard is $7 million. This solicitation is full and open competition. • V. V. CONTRACT LINE ITEMS (CLINS), QUANTITIES, UNITS OF MEASURE : Please see solicitation Attachment A, Section 1. • VI. VI. DESCRIPTION OF REQUIREMENTS : In support of the Department of Homeland Security (DHS), U.S. Customs and Border Protection's (CBP), Directive NO. 51410-023 Physical Fitness Program's (Pilot) goal to create and support a culture within CBP where physical fitness is promoted across CBP, the Office of Technology Innovation and Acquisition (OTIA) seeks a contract with a fitness center to support and promote employees health and wellness. The contractor shall provide access to physical fitness facilities for CBP/OTIA uniformed service and government personnel assigned to 1901 South Bell Street, Arlington, VA 22202 (Crystal City Area Location Only). If contractors have other facilities in the National Capital Region, personnel will be encouraged to visit those locations primarily during non-working hours, but the primary emphasis for this procurement will be on the fitness center location or locations with accessibility to employees whose work location is at 1901 South Bell Street, Arlington, VA 22202. The Government intends to issue a firm-fixed-price Purchase Order type contract with an award term of one base year with four (4) one-year option periods for a possible contract length of 5 years. The Government will not purchase individual memberships. Rather, the Government will purchase unlimited access for up to 100 active members and will pay the same price every month, regardless of the number of active members. In each of the 5 potential years of this contract, in addition to purchasing unlimited access for up to 100 active members, there will also be 2 priced options. Each option, if exercised, will allow the Government to purchase unlimited access for up to an additional 25 active members. I f the Government exercises the option for up to 100 members plus one option for up to an additional 25 active members in any option year, then up to 125 active members will have unlimited access for that option year. If the Government exercises the option for up to 100 members plus both options for 25 additional members, then up to 150 active members will have unlimited access for that option year. Statement of Work Please see solicitation Attachment B, Statement of Work for the requirements. •VII. VII. DATES AND PLACES OF DELIVERY/ACCEPTANCE AND FOB POINT : Not Applicable •VIII. VIII. FAR 52.212-1, INSTRUCTIONS TO OFFERORS - COMMERCIAL ITEMS : FAR 52.212-1, Instructions to Offerors, Commercial, applies to this acquisition. Additional instructions include: •· Contractors shall return with their quotes the Solicitation #: HSBP1014Q0064 Pricing Template attached to this solicitation. •· Contractors shall specifically provide information in their quotes relative to the qualification and certifications of their staff and instructors such as CPR certification and nationally recognized instructor and certification degrees such as ACE, AFAA, ACSM. • IX. IX. FAR 52.212-2, EVALUATION - COMMERCIAL ITEMS : FAR 52.212-2, Evaluation - Commercial Items applies to this acquisition as follows: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Accessibility of fitness facility to 1901 South Bell Street, Arlington, VA 22202 (2) Technical (3) Price (4) Past Performance. Relative Importance of Evaluation Factors : Accessibility of fitness facility to 1901 South Bell Street, Arlington, VA 22202 is more important than technical. Technical is more important than past performance. Past performance is more important than price. All non-price factors, when combined, are approximately equal to price. Accessibility of fitness facility to 1901 South Bell Street, Arlington, VA 22202 Evaluation Factor Defined : Accessibility includes, but is not limited to: •· Physical distance from 1901 South Bell Street, Arlington, VA 22202: •· Method or methods to get to and from the fitness facility (walk, inside/outside, train, bus, shuttle, employee car, etc.); and •· Estimated time it is expected to take the OTIA employee to travel to and from the facility Technical Evaluation Factor Defined : Includes, but is not limited to (not listed in order of importance): * Facility hours; * Total number and locations of facilities available for use by OTIA employees; * Qualified Staff and Instructors including CPR certification and national recognized instructor and certification degrees such as ACE, AFAA, ACSM; * Variety and quantity of cardiovascular/aerobic exercise equipment; * Variety and quantity of weight training machines and equipment; * Variety and quantity of fitness/resistance exercise equipment; * Variety and quantity of exercise classes; * Locker rooms and shower facilities; and * Other amenities or services. Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. • X. X. FAR 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS : FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, applies to this acquisition. Offerors shall include a completed copy with its offer. A blank copy can be found in solicitation Attachment A, Section 2. • XI. XI. FAR 52.212-4, CONTRACT TERMS AND CONDITIONS : FAR 52.212-4, Contract Terms and Conditions applies to this acquisition. In accordance with FAR 52.212-4(i)(1), the Government will not prepay for services not yet delivered to the Government or accepted by the Government. The Government will pay the contractor on a monthly basis in arrears. For example, for the January 1 through January 31 monthly timeframe, the contractor shall invoice the Government no earlier than February 1 - after services have been delivered to the Government. •XII. XII. FAR 52.212-5, CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS - COMMERCIAL ITEMS : FAR 52.212-5 applies to this acquisition. See Attachment A, Section 3 for specific clauses that apply. •XIII. XIII. OTHER CONTRACT TERMS AND CONDITIONS NECESSARY FOR THE ACQUISITION : See Attachment A, Section 4 for other terms and conditions that apply to this acquisition. •XIV. XIV. DEFENSE PRIORITIES AND ALLOCATIONS SYSTEM (DPAS) : DPAS is not applicable for this procurement. •XV. XV.
