- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) 246-2014-Q-0011. This procurement is 100% small business set-aside under NAICS code 339115 with a standard business size of $6.5M. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72, effective January 30 2014. Estimated start date is June 15, 2014. The closing date for receipt of quotes is May 2, 2014. Offers shall be submitted to the Oklahoma Area Indian Health Service, 701 Market Drive, Oklahoma City, Oklahoma 73114, no later than 4:30 p.m., on May 2, 2014. The offer must be submitted in a sealed envelope, addressed to this office, showing the solicitation number, and your name and address. Please do not email RFQ's. Qualified vendors will review the following and submit applicable information. No phone inquiries. EYE GLASS PROGRAM Clinton Indian Health Center Clinton, Oklahoma General 1. The contractor shall provide spectacle frames and lenses to include scratch resistant coatings on all plastic lenses and a 1 year warranty on frame and lenses at no additional charge. 2. The contractor shall supply 350 frames for display in the eye clinic. Frame display will include a variety of mid range frames suitable for men, women, children, sports recreational specs and unisex. Frames will be supplied and updated at no charge to the Clinton Indian Health center 3. Additional frame displays will be provided at no charge if needed. Services to be provided by the Contractor 1. Lenses will be fabricated to the exact specifications provided by the eye clinic. 2. Dr,s rx changes will be provided at no additional charge within 90 days of the original order 3. All frames and lenses will include a 1 year warranty based on industry standards. 4. The contractor will bill Contract Health Services directly on a monthly basis 5. Completed frame and lens orders will be delivered by mail to the eye clinic or directly to the patient as the eye clinic specifies. 6. There will be no additional charge to the package pricing for oversize, high powers or prisms. 7. Orders will be delivered within 14 days.
