This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (1) The solicitation number and a statement that the solicitation is issued as a request for proposal VA259-14-AP-3238. (2) The associated NAICS 325412 code and is set-aside for small business size standard 750 employees. (3) Seeking to purchase BIOMED Baxter Spectrum Wireless battery modules P/N 35083, quantity of 100 each. Delivery of items: VA Healthcare System 500 Foothill Drive, Slat Lake City UTAH 84148-0001. (4) Provision at 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition, 52.212-2, Evaluation -- Commercial Items, evaluation criteria shall be based on price and delivery (5) Offeror shall include 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. (6) Clause 52.212-4, Contract Terms and Conditions -- Commercial Items, and 52.212-5 Contract Terms and Conditions apply to this acquisition.
