Description

Statement of Work 1.Title - Service on station to repair Nidek Confoscan 4, serial# CS4312 2.Scope - Vendor will provide all labor, parts, and travel to repair Confoscan on site at the VA Hospital Eye Clinic in Little Rock, AR. Must repair Confoscan back to normal operating condition. 3.Background - The screen on the Confoscan is jumping and the images are distorted. 4.Performance period - Vendor is to begin repairs as soon as purchase order is issued. 5.Type of Contract - Firm Fixed Price 6.Evaluation factors - Contractors must be able to: a.OEM trained service technician to perform repair b.OEM approved replacement parts c.Detailed service report
