- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The solicitation number is VA262-14-Q-0204, and this solicitation is a Request for Quote (RFQ). This Total Small Business Set-Aside announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The Department of Veterans Affairs, Network Contracting Office (NCO) 22, on behalf of the VA San Diego Healthcare System (VASDHS), has a need for a new medical grade, large screen monitoring management system, and associated installation, training, and warranty. Specific requirements are listed on the attached Schedule of Supplies and Services, brand name or equal, and salient characteristics on the Statement of Work (hereafter referenced as SOW). The North American Industrial Classification System (NAICS) code for this requirement is 339112 with a size standard of 500 employees. This acquisition is a 100% Total Small Business Set-Aside. The Contractor shall adhere to all federal and state laws and regulations in effect during the term of this contract. The Government intends to award a firm-fixed-price contract. The contract will be effective on the date of award, and the requirements of the Schedule of Supplies and Services and SOW must be completed no later than August 7, 2014. Offerors should thoroughly review the Schedule of Supplies and Services and SOW and be familiar with the requirements of the solicitation prior to submitting quotes in order to be fully aware of the scope of services required. Failure to do so will not relieve the successful offeror from performing in accordance with the strict intent and meaning of the specifications without additional cost to the Government. The Government shall not reimburse any cost not incorporated into the offeror's price. Respond to the requirements of the SOW by providing a written document that is no longer than ten (10) pages. Complete the blanks on the attached Schedule of Supplies and Services, and Offerors should include their local part numbers in the appropriate space on the form. Also, complete the attached Contractor Qualifications Questionnaire, and complete the attached FAR Part 53.212-3 Offerors Representations and Certifications-Commercial Items. All documents must be submitted in an MS Word or Adobe readable format. All documents shall be single spaced, use Times New Roman font, in eleven (11) pitch, and have a 1" margin all around. Offerors shall include a table of contents. Email the completed quotes to Susanne.Christen@va.gov by the response date and time listed in this combined synopsis/solicitation. Please include "VA262-14-Q-0204" in the subject line of emails. Vendors must honor quotes for thirty (30) days from the closing date of this RFQ. Technical inquiries may be emailed to Susanne.Christen@va.gov. Technical inquiries will be accepted through 1 p.m. PST on May 2, 2014. Please include "VA262-14-Q-0204" in the subject line of emailed inquiries. Telephone inquiries will NOT be accepted. No hard copies of this RFQ will be provided. CLAUSES, PROVISIONS, AND ADDITIONAL INSTRUCTIONS 1. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-60. 2. The following provisions and clauses apply to this acquisition: FAR Parts 52.202-1, 52.211-6, 52.212-1, 52.212-2, 52.212-3, 52.212-4, 52.212-5, 52.219-6, VAAR 852.203-70, 852.211-70, 852.211-70B, 852.232-72, 852.246-70, 852.246-71. 3. FAR Part 52.212-1, Instructions to Offerors-Commercial and FAR Part 52.212-4 apply to this acquisition and no addenda are attached. 4. FAR Part 52.211-6, Brand Name or Equal and FAR Part 52.219-6, Notice of Total Small Business Set-Aside applies. 5. Procurement for these commercial services is being conducted according to FAR Part 12, 13, 19, Commercial Items, Simplified Acquisition Procedures, Small Business Programs, and Veteran Affairs Acquisition Regulations as supplemented with additional information in this notice. 6. BASIS FOR AWARD: Subject to the provisions contained herein, award shall be made to the Offeror deemed responsible in accordance with the Federal Acquisition Regulation and who conforms to the solicitation requirements; and whose proposal, judged by an overall assessment of the Offeror's qualifications specified in this solicitation, meets the technically acceptable standard for the non-cost factors and provides the lowest evaluated price. Non-cost factors are significantly more important than price. The Department of Veterans Affairs (VA) intends to issue a Firm Fixed-Price Delivery Order for commercial supplies. Note: Offeror shall submit a completed Contractor Questionnaire to be considered for award of this solicitation. 7. THE GOVERNMENT RESERVES THE RIGHT TO AWARD THE CONTRACT WITHOUT DISCUSSIONS. QUOTES SHALL ONLY BE ACCEPTED FROM SMALL BUSINESSES. 8. Offerors shall include a completed copy of the required FAR Part 52.212-3, Offeror Representations and Certifications-Commercial Items, with the submittal. See attached documents: SOW, Schedule of Supplies and Services, Contractor Questionnaire, and FAR Part 52.212-3, Offer Representations and Certifications-Commercial Items.
