*****Begin Text Document- 'FBO-DESCRIPTION' ***** This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement is being issued as a request for quotation (RFQ) and constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular #2005-71 Effective 25Nov2013. This combined synopsis/solicitation is being competed on a 100% Service Disable Veteran Owned Small Business (SDVOSB) set aside. This acquisition will be competed to the full extent required by regulation. The NAICS code that applies is 562910 and business size millions of dollars is $14 Million. This requirement is to Abatement and Removal Services. Each quote will be evaluated in accordance with the Meet Or Exceed Standard. All quotes must be entered on SF1449 Solicitation document. ALL OFFERERS SHALL SUBMIT THE CONTRACTING OFFICER THE FOLLOWING DOCUMENTATION (Basis of Award) **ALL factors must be rated ACCEPTABLE to be considered for award. Non-Cost/Price Factors 1.TECHNICAL INFORMATION: Each offeror shall describe their plan to accomplish the following technical criteria as specified in the Statement of Work: a.Contractor shall provide Asbestos license and training documentation b.Contractor shall provide all necessary equipment and tools along with PPE( Personal Protection Equipment that meet OSHA requirements for safety (OSHA Standard 29 CFR 1926.1101). c.Contractor shall provide written equipment list for respirator protection plan that includes fit testing and medical evaluations. d.Contractor shall provide written equipment list for abatement equipment and materials. e.Contractor shall be registered in VetBiz see Acquisition Flash page:16. f.Contractor shall have a written plan to complete the requirement In accordance with VA specification 02 82 13.19 ASBESTOS FLOOR TILE AND MASTIC ABATEMENT g. 2. CAPABILITY (WRITTEN): Each offeror shall, address the following capability criteria: a.Organizational Experience: The offer shall address their related work experience of similar scope, magnitude and complexity to that described in the solicitation. This information shall include the following: b.Company's history, organization, qualifications and work experience relating to the Statement of work c.Contracts similar in magnitude and complexity to the work anticipated under this requirement. d.Annual volume of workload. e.Management Plan: The offeror shall address their plan for providing services in accordance with the solicitation, including how they maintain efficiency and quality control of services for their customers. As a minimum, the offeror shall describe the details for implementing, managing and assuring satisfactory performance and compliance with the Statement of Work. 3.Relevant Experience/Past Performance - The Relevant Experience/Past. Performance evaluation will assess the relative risks associated with an offeror's likelihood of success in performing the solicitation's requirements as indicated by that offeror's record of Relevant Experience/Past Performance. "Offerors must provide at least two (2) references and two (2) successfully completed contracts that demonstrate previous work experience in asbestos abatement in healthcare facilities for a minimum of three (3) continuous years. Provide description of projects, dollar value, and Point of Contact(s) including phone/FAX numbers. This is a best value acquisition. The Government will make an award to the responsible offeror whose offer conforming to the solicitation is the most advantageous to the government.
