- ITEM UNIQUE IDENTIFICATION (IUID) CHECKLIST Transportation Data DD Form 1653 Packaging Requirements AFMC Form 158 Solicitation FA8524-14-R-0004 This acquisition is to obtain a firm fixed price requirements contract for the purpose of purchasing spares for the Swing Arm Flap Attachment applicable to the C-5 aircraft, NSN 1560-00-260-0156UC (Part Number: 4J33150-107A). The requirements contract will have a Basic performance period of two (2) years plus three (3) annual options. The Basic period will require a First Article. Delivery of First Article is to be within 365 days of award of contract. No work on the production quantity is to begin until receipt of the approval of the First Article. The BEQ for the Production Quantity for the Basic period will be 64 each. The BEQ for Option I is 14 each. The BEQ for Option II is 10 each. The BEQ for Option III is 10 each. Delivery is to be within 240 days of receipt of approval of First Article or Production Unit Delivery Order, whichever is later. Destination: The First Article and Production Quantities are to be delivered to Robins AFB, GA. This is a Total Small Business Set Aside as implemented by FAR 19.5. Data or rights are owned by the Government for this solicitation. Approximate issue date of solicitation is 23 April 2014. Approximate response date is 23 May 2014.
