Attachment 2 - Scope of Work Attachment 1 - Schedule of Price Questions and Answers (i) This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in subpart 12.6 of the FAR and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. (ii) Solicitation number IHS-14-HQ-RFQ-00026 applies, and is issued as a Request for Quote (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. (iv) The North American Industry Classification System (NAICS) code is 611430 with a small business size standard of $10M. This acquisition is set-asided for small businesses only. (v) Fill out the attached, Attachment 1, Schedule of Prices. Period of Performance: one year base period and (4) one year option periods. (vi) The IHS Division of Nursing has been tasked with facilitating a key national initiative, the IHS Baby-Friendly Hospital Initiative (BFHI). This quality improvement initiative is to increase breastfeeding rates, thereby creating a healthy start on life and preventing childhood obesity. In order to be designated Baby Friendly, maternity facilities must adopt the BFHI's Ten Steps for Successful Breastfeeding, a specific set of evidence-based practices that support breastfeeding initiation and sustained breastfeeding over time. A component of the certification process is the training of employees that reflects compliance with Baby-Friendly guidelines and criteria. See the attachment 2, Scope of Work for more details. (vii) The delivery and acceptance term is FOB Destination. (viii) FAR 52.212-1 - Instructions to Offerors - Commercial Items (Jul 2013) Quotes shall be submitted on company letterhead stationery, signed and dated; it shall include: (1) The solicitation number; (2) The name, address, and telephone number of the offeror; (3) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. (4) Price and any discount terms; (5) "Remit to" address, if different than mailing address; (6) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically); (7) Acknowledgment of Solicitation Amendments. (ix) 52.212-2, Evaluation - Commercial Items (Jan 1999) Commercial Items is applicable to this requirement. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Technical Evaluation, Relevant Past Performance and Price. All technical factors, when combined, are more important than Price. The offerors must respond to the technical evaluation criteria and provide a firm fixed price quote for each year. The Government intends to evaluate offerors and award a contract without discussions with offerors. Therefore, the initial offer should contain the offerors best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if later it is determined by the Contracting Officer to be necessary. The Government may reject any or all offers, waive informalities and minor irregularities in offers received. The offerors must include itemized price lists of charges, complete description of services, complete address for invoice payment and prompt payment discount terms, the Dun & Bradstreet Number (DUNS), the taxpayer Identification Number (TIN) and certification of business size of the company. The evaluation criteria are listed below: (1) Technical Approach - Offerors shall submit a list of trainings to be offered. The quote must evidence that the educational program will provide continuing nursing education credits that are issued by a recognized nurse accrediting body and the International Board of Lactation Consultant Examiners. In addition, the quote must describe the offeror's experience, knowledge and skills related to the Baby-Friendly Hospital Initiative Ten Steps. Submission of Technical Approach package is limited to 10 pages. (2) Past Performance - Offerors are required to provide corporate resumes for no less than three (3) and at most five (5) contracts performed during the past seven years or currently in process for the relevant services as are being solicited under this solicitation. The Vendor's relevant past performance information must include the following; Contract Number(s); type of contract; period of performance; total original, present, or final contract dollar value; and specify whether the Vendor was the prime Contractor or the subcontractor. A representative of IHS may contact the offeror's previous customer(s) for a phone interview to help rate the vendor's performance. The Government may supplement the information vendors provide with performance information it may obtain from any source including its own experience with the vendor. Submission of Relevant Past Performance information is limited to 3 pages.
