- I. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with no additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number is 0040141549. This issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. II. This procurement is for 1.5 inch crushed concrete gravel to be delivered and stockpiled/tailgate spread to various locations. III. This requirement is being procured in accordance with FAR Part 12, Acquisition of Commercial Items and an award will be made in accordance with FAR Part 13, Simplified Acquisition Procedures. This action is being solicited as a small business set-aside and only qualified offerors may submit a quote. The NAICS Code associated with this requisition is 212321 with a small business size standard of 500 employees. Buyer intends to issue award using a firm fixed priced purchase order. Bids from offerors unable to accept purchase orders will not be considered for award. IV. Offerors are cautioned that a warranted Contracting Officer (CO) is the only authorized Government Representative to make any contractual changes once an award is made. No oral statement of any person whomsoever shall, in any manner or degree, modifies or otherwise affects the terms of this procurement. The CO for this order is Jamese Promise; jamese_promise@fws.gov. V. Government Representative (GR): Authorized Government Point of Contact for this order is Sharon Anderson; sharon_anderson@fws.gov. She has no authority to make any changes in this procurement. VI. Description: (1.) Line Item 0010: Provide 2950 cubic yard of crushed concrete gravel material delivered to various locations to be stockpiled and/or 6 inch spread; as specified. Special Notice: MUST provide current gradation report reflecting sieve, 1.5 sampling meets at 100. No report submitted will result in being disqualified for consideration. This work entails furnishing all necessary management, supervision, labor, materials, and equipment to supply 2950 cubic yard of crushed concrete gravel for delivery and spread with a minimum of 10 deliveries per day. This will be under the direct supervision of Refuge Staff. The contractor shall ensure total for all amounts supplied does not exceed the amount noted in the contract line items. VII. Delivery Requirements: This is an estimate and is subject to change. (1.) 500 cubic yards delivered to Grand Cote National Wildlife Refuge; stockpile (2.) 1250 cubic yards delivered to Lake Ophelia National Wildlife Refuge; stockpile (3.) 1200 cubic yards delivered to Lake Ophelia National Wildlife Refuge; tailgate spread Coordinate with Refuge staff about exact delivery quantities and exact locations for this is subject to change. Due to the remote locations, the contractor will be expected to assign more than one truck at a time to deliver on the site. The location is too remote to receive one load a day. A minimum of 10 deliveries would be expected daily. Material can be delivered utilizing trucks with 24 yard trailers. Note: There may be county road weight limit restrictions coming into this site the vendor needs to be aware of. The vendor would need to verify with the county road commissioner if a bound is required to haul on this road. The technical point of contact Brett Wehrle, brett_wehrle@fws.gov; 318-253-4238. Once delivery begins, the Contractor shall deliver at a rate of not less than 10 truckloads per day during regular working hours. Hours are 7:00 A.M. to 3:30 P.M. The delivery will accompany a tailgate spread or spot dump with amount piled and thickness to be determined by the site manager at the designated location. Identified delivery location will be at designated through site manager. All deliveries will occur within an active jobsite or staging area that will accommodate highway compliant vehicles, required delivery trucks, etc. Drivers shall furnish certified scale tickets to FWS/government personnel upon delivery of each load to the delivery site. Required commencement date for delivery is on or about 7 days from award date. Last day for delivery is September 30, 2014. For inquires on delivery site details, exact locations and specifications, please reference attached Roadway/Gravel Surface Material sheet and map or contact technical point of contact Brett Wehrle. NOTE: It is the Contractors responsibility to visit the site prior to bidding and know existing delivery and dumping conditions. VIII. Local conditions (bridge restrictions, weather conditions, water levels, road flooding, etc.) that may affect delivery: None. Contractor shall coordinate deliveries (including hours of delivery) 48 hours in advance with the Government Representative. IX. System for Award Management (SAM) Registration - A prospective awardee shall be registered in the SAM database prior to award. Information on registration may be obtained via the Internet at: https://www.sam.gov/portal/public/SAM/#1 or by calling 866-606-8220. Failure to maintain a current/active account so will render your offer non-responsive. X. RFQ Due Date: Telephone quotes will NOT be accepted. All quotes must be received at this office no later than May 22, 2014, 11:00 AM Local Eastern Time. ONLY EMAILED QUOTES WILL BE ACCEPTED. Email to constance_fields@fws.gov. Attention to: Constance Fields. Please reference/subject: solicitation number. Quotes should be attached document that is either Microsoft Word 97 compatible or PDF. The anticipated award date on or about Thursday, May 29, 2014. Please do not request award status prior to this date. XI. All interested Contractors shall provide a quote as follows: (1) SF 1449 is acceptable for submitting quote. Parties responding to this solicitation may also submit their offer in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc.), but at the minimum include the following information: (2) Line Item 0010: 1.5 inch crushed concrete gravel. Price per cubic yard $________; Total Price $_______. (3) Gradation report reflecting sieve 1.5 sampling meeting at 100. No report submitted will result in being disqualified for consideration. (4) Company's name, point of contact, address, phone number, fax number, email address, solicitation number, item Description/Specification (Descriptive literature if applicable), total price (itemize price final total price),and approximate daily delivery schedule ( include number of trucks, amount to be delivered, and total number of days to complete requirement). Price(s) should be for FOB Destination. (5) A list of three (3) most recent past performance references for a similar purchase including name/point of contact, address, phone number, and email address. (6) Statement confirming registration in SAM (www.sam.gov). Statement confirming Representation and Certification completion within SAM. Provide account expiration date, cage code, DUNs number, and taxpayer ID number. (7) Statement confirming that you will accept electronic payment through IPP (www.ipp.gov). (8) Quote submission: All quotes submitted shall be good for 30 days after RFQ closed date. (9) Limited Area: Contractors wishing to bid on this posting must reside within a 60 mile radius of Lake Ophelia National Wildlife Refuge, 71351; in order to be deemed responsive. Vendors outside of the 60 mile radius area, with suppliers who are inside of the 60 mile radius area required for this posting, may submit a bid. Include local vendors name, vendor representative name, address, email address, and phone number in quote. (10) Evaluation Criteria/Basis of Award: Pursuant to applicable acquisition regulations and/or departmental guidelines, Buyers may use criteria other than price to evaluate offers. Accordingly, please note that, unless otherwise specified in the Buy Terms, to the extent required by applicable regulations and/or guidelines, award will be made to the responsible Seller whose offer conforming to the solicitation will be most advantageous to the Buyer on the basis of price, technical capability, delivery, and past performance. NOTE: Any quotes not received by the due date and time specified in this synopsis/solicitation will be rejected. Additionally, any incomplete quote not containing the required information as stated in this solicitation may also be rejected. NOTE: The final quoted price(s), both total and individual, MUST include all applicable fees, gratuities, commissions, service charges, etc. THE FEDERAL GOVERNMENT IS TAX EXEMPT.
