*THE PURPOSE OF THIS AMENDMENT IS TO CHANGE FROM 100% SMALL BUSINESS SET ASIDE TO UNRESTRICTED ACTION. This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation; quotations are being requested and a written solicitation will not be issued. The solicitation number is N00259-14-T-0149. It is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. This is an unrestricted action. The North American Industry Classification System (NAICS) Code for this acquisition is 811420. The size standard is $7.0M. It is the contractor's responsibility to be familiar with the applicable clauses and provisions. This Statement of Work (SOW) covers the standards for the installation and upholstery of government owned furniture. 1.1 Background. 1.1.1 The intent of this scope is to guide the contractor in installation standards for furniture upholstery in the clinics at Robert E. Bush Naval Hospital, Twentynine Palms, California. 2 LOCATION. 2.1 Clinics Area, Building 1145, Robert E Bush Naval Hospital Twentynine Palms, Marine Air Ground Task Force training Center, Marine Corps Air Ground Combat Center, Twentynine Palms, CA 92278. 2.1.1 Clinic A - Mental health, Nutrition Clinic 2.1.2 Clinic B - Obstetrics/Gynecology, Family Medicine - Blue Team 2.1.3 Clinic C - Family Medicine - Gold Team 2.1.4 Clinic D - Pediatrics/Immunization, Optometry 3 REQUIREMENTS. 3.1 General. 3.1.1 The contractor shall provide labor and material to upholsterer government owned furnishings within the clinic reception areas. Installation will not be considered satisfactory until the end users Infection Control representative has conducted a walkthrough and inspection following product installation. 3.1.2 The government owned furnishing to be upholstery are as follows: 3.1.2.1 Quantity 35; Garrett Loveseats 3.1.2.2 Quantity 41; Arris Chairs 3.1.2.3 Quantity 6; Arris Bariatric Chairs 3.1.2.4 Quantity 3; Capital Glider with locking mechanism 20" Seat 3.2 Detail tasks. 3.2.1 Contractor shall make maximum effort to minimize disruption to patient care within the hospital 3.2.2 Details shall be coordinated with the Facilities Management representative, Housekeeping Supervisor, Infection Preventionist, and Director of Medical Services three weeks prior to installation to establish a phased project schedule for project. 3.2.3 Contractor shall verify with the representative the areas which will have furniture upholstery per the applicable phase. 3.2.4 Contractor must verify with the Facilities Management representative the point of entry for installation, and any requirements for material lay down or storage. 3.2.5 Contractor to provide 24 hour notice to Facilities Management representative prior to start of installation. 3.2.6 Parts and Supplies provided under this contract shall be new. 3.2.7 Upholstery to consist of the following qualification: 3.2.7.1 Maintenance 3.2.7.1.1 Mildew resistance, antibacterial, antimicrobial, antifungal, and stain resistance material 3.2.7.1.2 Cleans with water; water-based cleaning agents or foam and solvents, Bleach cleanable (4:1) 3.2.7.2 A rating of at least no evidence of stain present 3.2.7.3 The upholstery will meet Flame Resistance set by Federal, State and/or Local regulations: 3.2.7.3.1 Meets or exceeds: 3.2.7.3.1.1 Flammability: CAL 117 D SEC E 3.2.7.3.1.2 Flammability: UFAC/NFPA260 3.2.7.3.1.3 California Technical Bulletin 133 3.2.7.4 Durability of at least 365,000 RUBS 3.2.7.5 Color of Dark Blue or equivalent 3.2.7.6 Will Meet or exceed standard physical properties for coasted fabric set by ASTM International 3.2.8 Contractor will be required to contact the Facilities Department representative to submit the required information to complete the Security Pass Form by 1400 the day prior to the scheduled visit. 3.2.9 Contractor is required to report to the Facilities Department staff, Robert E Bush Naval Hospital Twentynine Palms, for Visitor Pass prior to and upon completion of any service/repair performed. 3.2.10 Contractor shall comply with all infectious control rules for this project. 3.2.11 Contractor will be responsible for cleanup of all packing materials, crates, crating, and/or other debris created in completion of the purchase order. 3.2.12 Contractor shall be provided reasonable access to all equipment that is to be serviced and utility outlets required to do the service. 3.2.13 Contractor may use Government utilities, (e.g., electrical power, compressed air, and water) that are available and required for any service performed under this contract. The building engineer, to ensure compatibility with the Naval Hospital's electrical wiring and equipment, must approve contractor electrical equipment. 3.2.14 Contractor shall adhere to submitted contract; any and all changes by the Contractor must receive written approval and direction prior to performance. 3.2.15 Contractor shall perform all services necessary to complete the contract in a satisfactory and acceptable manner in full compliance with specifications and terms of the contract and adhere to all applicable Federal, State, and Local regulations, including all environmental regulations. If additional services are required other than what is listed on scope of work, the Contractor shall notify the Department Head, Facilities Department, Naval Hospital Twentynine Palms of additional service changes required. 3.2.16 All work must be performed during the hours of 0730 to 1600 and must be coordinated with the Facilities Department representative. 3.2.17 Government will be responsible to set-up safety cones to identify working areas. 3.2.18 Government will be responsible to identify government owned equipment to be upholstered.
