Description

SOURCES SOUGHT ANNOUNCEMENT: The Department of Veterans Affairs Lebanon VA Medical Center, 1700 South Lincoln Avenue, Lebanon, PA 17042 is conducting a market survey to identify potential sources to provide on call Nursing or Certified Patient Care Technician support services for acute inpatient hemodialysis. Care shall include initiation, monitoring and discontinuation of acute hemodialysis treatments. This announcement is issued for informational planning purposes only and is not a request for proposal or announcement of a solicitation. Submitting information for this announcement is voluntary and participants will not be compensated. The purpose of this market research is to identify capable sources of providing 24/7 on call patient care services for support of acute hemodialysis. Nurses or Certified Technicians providing these services would be required to be fully trained in and qualified as a Hemodialysis Care Technician and possess a valid Basic Life Support (BLS) certification. Care takes place in an Intensive Care Unit (ICU) setting. The maximum on call response time is within six hours of notification. The VA Lebanon Medical Center currently utilizes a Phoenix Dialysis system. The North American Industry Classification System Code (NAICS) is 621399 and the small business size standard is $7.0 Million. Capabilities Statement and Documentation Firms/Companies are invited to provide their capabilities, experience and knowledge in the provision of Nurses and/or Certified Technicians to provide hemodialysis treatments. Sources are asked to provide information to demonstrate their answers to the following items of information: 1.Company information; Address, Website, POC, and POC Phone Number. 2.The North American Industry Classification System Code (NAICS) is 621399 and the small business size standard is $7.0 million for this type of effort. Based on this information, please indicate whether your company would be a Large or Small Business, and provide the average amount of sales for the last three (3) years. 3.A summary demonstrating ability to provide patient care services directly related to acute hemodialysis care. 4.A summary of firm's ability to provide Nurses or Certified Technician staff with the requisite certifications and skills/experience. 5.Indicate any staff experience with Phoenix Dialysis Systems. In addition to a Capability Statement which provides the above, submissions can include specifications, brochures, and manuals as attachments.
