- The Department of Veterans Affairs, Central Arkansas Veterans Healthcare System (CAVHS), 4300 West 7th Street, Little Rock, AR, intends to negotiate procurement a replacement and install of an dock leveler for the Department of Veterans Affairs Medical Center, 921 N.E. 13th Street, Oklahoma City, Oklahoma, 73104. GENERAL A.The contractor shall furnish labor, equipment and materials and perform the required center dock leveler replacement at receiving dock for the Department of Veterans Affairs Medical Center, 921 N.E. 13th Street, Oklahoma City, Oklahoma, 73104 as required by this statement of work. B.Visits to the site by bidder may be made only by appointment with the warehouse supervisor, Jessie Cochran at telephone number (405) 456-4845. C.The Contractor shall confine all operations (including storage of materials) on Government premises to areas authorized or approved by the COR. The Contractor shall hold and save the Government, its officers and agents, free and harmless from liability of any nature occasioned by the Contractor's performance. D.Execute work so as to interfere as little as possible with normal functioning of Medical Center as a whole, including operations of utility services, fire protection systems and any existing equipment and with work being done by others. Use of equipment and tools that transmit vibrations and noises through the building structure, are not permitted in buildings that are occupied, jointly by patients or medical personnel, and Contractor's personnel, except as permitted by COR where required by limited working space. E.Phasing: To ensure such executions, Contractor shall furnish the COR with a schedule of approximate dates on which the Contractor intends to accomplish work in each specific area of site, building or portion thereof. In addition, Contractor shall notify the COR two weeks in advance of the proposed date of starting work in each specific area or site, building or portion thereof. Arrange such dates to ensure accomplishment of this work in successive phases mutually agreeable to Medical Center Director, COR and Contractor. F.Buildings will be occupied during performance of work. 1.Contractor shall take all measures and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris, so that equipment and affected areas to be used in the Medical Center's operations will not be hindered. 2.No utility service may be interrupted without prior approval of COR. 3.Disposal and Retention: Materials and equipment accruing from work shall be disposed of by the Contractors. II. PROJECT SCOPE A.Schedule all work in specific areas with the COR to enable Logistics to make any necessary arrangements. Coordinate with the COR in advance of any outages. Only after the approval of the schedule outages, by the COR, work can proceed accordingly. B.All work to be started and completed within 1 Saturday only to alleviate any logistical delivery interruptions to medical center. C.Dock leveler and Lighting replacement will be, in general, of the following equipment: " Aftermarket Hydraulic Conversion Kit "? ¾ HP Pump and Motor Assembly "?
