Description
This is a SOURCES SOUGHT NOTICE only. Responses to this notice will be used for information and planning purposes. No proposals are being requested or accepted with this notice. THIS IS NOT A SOLICITATION FOR PROPOSALS AND NO CONTRACT SHALL BE AWARDED FROM THIS NOTICE. This notice shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). Responses to this sources sought is not a request to be added to a prospective bidders list or to receive a copy of the solicitation. The VA Gulf Coast Veterans Health Care System (VAGCVHCS) has been tasked with finding a Provider/Offeror in the Mobile, AL area only to provide for the following Neuropsychological services: I.) A Full Neuropsychological Evaluation involves clinical interview, review of available medical records, an assessment of the following cognitive domains: 1. General Intellectual Ability 2. Attention/Concentration 3. Working Memory 4. Information Processing Speed 5. Language 6. Secondary Memory (verbal and non-verbal, as needed) 7. Visuospatial Functioning 8. Executive Functioning 9. Optional (as needed to answer referral question) a. Functional Ability (PSMS, IADLS, Independent Living Skills) b. Motor Functioning (Motor Speed and Fine Motor Coordination) c. General Achievement Instruments d. Measures of Emotional/Psychological Distress e. Stand-alone Symptom Validity tests Finally, conduct a profile analysis regarding the aforementioned domains, and generate an integrative report that includes summary with diagnostic impression, and patient specific recommendations. II) A Neuropsychological Screening involves clinical interview, review of available medical records, and administration of one of the following two batteries: 1.RBANS, Trails, Clock Drawing, BDI-2 and the BAI, or 2.Neuropsychological Assessment Battery (NAB) Screening Module An Optional Stand-alone Symptom Validity tests may be administered as needed. And finally, conduct a profile analysis utilizing the aforementioned measures and generate an integrative report that includes summary with diagnostic impression and patient specific recommendations. The intent is to sole-source with the University of South Alabama if no other sources are available. To this end, Offerors must demonstrate ongoing capacity and resources to provide the services listed above. Offeror's response to this Notice shall be limited to 5 pages and shall include the following information: "Offeror's company name, DUNS, address, point of contact, title, phone number, and email address "Offeror's interest in bidding on the solicitation when it is issued "Offeror's capability to meet this requirement (may include literature detailing experience and resources) including any current contracts (Civilian or Government). "Offeror's type of business (small or large) and socioeconomic status whether Service Disable Veteran Owned, Veteran Owned, Hubzone, 8(a), Woman Owned, Small Disadvantaged, etc The NAICS for this is 621112, Offices of Physicians, Mental Health Specialists (size standard of $10.0 million). Responses must be received by no later than 4 pm Central time, May 12th, 2014. Responses may be emailed to Melanie.Grigsby@va.gov (former name Hawley) using "sources sought number VA256-14-R-0751" in the subject line. Facsimile responses will not be accepted. After review of the responses to this sources sought notice, a solicitation announcement may be published on the FBO website. Responses to this sources sought notice are not considered adequate responses to the solicitation announcement. All interested offerors will have to respond to the solicitation announcement in addition to responding to this sources sought notice. All interested parties are reminded to be registered with System of Award Management (SAM- formerly CCR) at http://www.sam.gov in order to be eligible for award of Government contracts.
