Description
General: This combined synopsis/solicitation VA-247-14-Q-0725 for commercial items has been prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This notice constitutes the only solicitation; a quote is being requested and a written solicitation will not be issued. A quotation in response to this notice must be received not later than 1:00pm EST Monday May 12, 2014. The solicitation number is VA247-14-Q-0725 and is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-30. No telephone requests will be accepted. Only written requests received directly from the requester are acceptable. It is the Offerors responsibility to ensure that the quote is received. The applicable North American Industry Classification System Code (NAICS) is 334210, Telephone Apparatus Manufacturing; the size standard is 1000 employees. OFFERORS MUST STATE THEIR BUSINESS SIZE when submitting their offer AND if the offeror has a FSS/GSA number, please include it on your offer. This procurement is being set aside 100% for Service Disabled Veteran Owned Small Businesses. 2. Set-Aside Requirement: Only verified SDVOSB and VOSB firms are eligible to submit an offer or receive an award of a VA contract that is set-aside for SDVOSBs or VOSBs in accordance with VAAR Part 819. A non-verified vendor that submits a bid or proposal should be rejected as non-responsive or technically unacceptable as listing in VIP is not self-correctable as a minor informality as, for example, a listing in SAM. All other purported SDVOSBs and VOSBs must apply for and receive verified status in accordance with 38 CFR Part 74 and be listed in VIP prior to submitting a bid or offer on an acquisition conducted in accordance with VAAR Part 819. The VIP database will be checked both upon receipt of an offer and prior to award. 2. Applicable Provisions and Clauses: This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-056. The following FAR and VAAR provisions and clauses are incorporate into this solicitation document by reference: 52.212-1, Instructions to Offerors-Commercial; 52.212-4, Contract Terms and Conditions - Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders (of which 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.223-18, 52.225-1, 52.225-13, and 52.232-34 are applicable); and VAAR 852.246-70, Guarantee. Additionally, please include a completed copy of provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with your offer. 52.212-2, Evaluation - Commercial Items, and VAAR 852.211-73, Brand Name or Equal, are also incorporated into this solicitation. At minimum, the offered item must meet all requirements set forth in the product specifications/salient characteristics section to be considered a responsive offer. The offeror that submits the offer that meets all the requirements of the solicitation, is responsive and responsible, and has the lowest cumulative price for all items (inclusive of delivery) will be selected. 5. Offeror Attachments If an "equal" item is offered, the offeror must submit descriptive literature that conclusively establishes product equivalency and technical acceptability and compatability at time of quote submission. Also, IF THE OFFEROR IS NOT THE MANUFACTURER OF THE QUOTED ITEMS, THE OFFEROR SHALL SUBMIT WRITEN EVIDENCE if the offeror is not the manufacturer of the quoted items, the offeror shall submit written evidence at time of quote that clearly verifies that the firm is an authorized distributor and/or reseller of the quoted products and an authorized installer/maintenance provider to ensure proprietary equipment, licensing, and warranties, both current and resulting from this procurement are available and not voided. 6. Offeror Instructions: The Government intends to issue a firm fixed price purchase order. Offerors shall submit quotes on Solicitation VA247-14-Q-0725. Quotes must be submitted electronically via email to Dino.LaRocca@Va.Gov. The email subject line shall reflect the following: "RFQ VA247-14-Q-0725 Quote Submission" New Equipment ONLY; NO remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. Bid MUST be good for 30 calendar days after close of Buy. Deliver to/Place of Performance:Hinesville CBOC 500 E. Oglethorpe Hwy Hinesville, GA. 31313 Delivery must be within 30 days ARO. FOB: Destination. This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information can be found at www.sam.gov. ? STATEMENT OF WORK Hinesville CBOC Activation 4/14/14 A.GENERAL INFORMATION 1. Title of Project: Unify Telephonic Items for Hinesville CBOC 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. The contractor shall assist the VA in programming a VA provided IPDA in Hinesville, GA. This quote is for 24 hours onsite programming to assist with installation. 3. Performance Period: The contractor shall complete the work required under this SOW in 30 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor's proposal, the contractor's proposed completion date shall prevail. Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO. 4. Type of Contract: Firm-Fixed-Price B. GENERAL REQUIREMENTS The contractor shall provide Flex License, Board for HG 3500 60 channel card, required labor for 4000 Custom MAC, and HiPath 4000 Power Redundancy Expansion. Warranty: The warranty shall commence after final inspection, completion of performance test, and upon full acceptance of the installation and shall concur with the guarantee period of service. During the warranty period, if a device is not functioning properly or in accordance with specification requirements, the equipment shall be removed and a new equipment meeting all requirements shall be installed as part of the work until satisfaction is obtained. Period of warranty shall start anew for such parts from date of completion of each new installation performed, in accordance with the foregoing requirements. C. CHANGES TO STATEMENT OF WORK Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor. D. TRAVEL [If applicable] All work is to be conducted at the Hinesville CBOC located at 500 E. Oglethorpe Hwy, Hinesville, GA 31313. The contractor is responsible for any anticipated travel and per diem. E. GOVERNMENT RESPONSIBILITIES [If applicable] The Hinesville CBOC personnel shall provide access to all secure areas that deemed necessary to complete the described work. F. CONTRACTOR EXPERIENCE REQUIREMENTS The contractor shall be currently and regularly engaged in the installation and service of the equipment and service indicated in this statement of work as one of their principle products. The contractor shall also have experience, trained supervisory personnel, equipment and facilities required to install and service the equipment and service indicated in this statement of work. G. CONFIDENTIALITY AND NONDISCLOSURE It is agreed that: 1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order. 2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response. 3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO. H. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS All contractor employees who require access to the Department of Veterans Affairs' computer systems shall be the subject of a background investigation and must receive a favorable adjudication from the VA Office of Security and Law Enforcement prior to contract performance. This requirement is applicable to subcontractor personnel requiring the same access. 1.Position Sensitivity - The position sensitivity has been designated as Low Risk. 2.Background Investigation - The level of background investigation commensurate with the required level of access is Minimum Background Investigation. 3.Contractor Responsibilities a.The contractor shall prescreen all personnel requiring access to the computer systems to ensure they maintain a U.S. citizenship, and are able to read, write, speak and understand the English language. b.The contractor shall submit or have their employees submit the required forms (SF 86 or SF 85P, SF 85P-S, FD 258, Contractor Fingerprint Chart, VA Form 0710, Authority for Release of Information Form, and Optional Forms 306 and 612) to the VA Office of Security and Law Enforcement within 30 days of receipt. c.The contractor, when notified of an unfavorable determination by the Government, shall withdraw the affected employee from working under the contract. d.Failure to comply with contractor personnel security requirements may result in termination of the contract for default. 4.Government Responsibilities a.The VA Office of Security and Law Enforcement will provide the necessary forms to the contractor, or to the contractor's employees, after receiving a list of names and addresses. b.Upon receipt, the VA Office of Security and Law Enforcement will review completed forms for accuracy, and forward the forms to the office of Personnel Management (OPM) to conduct background investigations. c.The VA Office of Security and Law Enforcement will notify the CO, and contractor, of adjudication results received from OMB. d.Upon being notified about a favorable determination, the CO may issue a notice to proceed to the contractor.
