General Decision Number: WI140021 02/07/2014 WI21 Superseded General Decision Number: WI20130021 State: Wisconsin Construction Type: Residential Counties: Barron, Bayfield, Burnett, Forest, Iron, Langlade, Lincoln, Oneida, Polk, Price, Sawyer, Taylor, Vilas and Washburn Counties in Wisconsin. HOME INTERIOR REMODELING SPECIFICATIONS STATION BAYFIELD GOVERNMENT OWNED HOUSING 1. PROJECT DESCRIPTION 1.1 The Coast Guard requires a Contractor fully qualified in general construction to complete repairs and remodeling in the government owned home located at 208 S. 10th Street, Bayfield, WI, 54814. The Contractor will be responsible for providing all labor, tools, equipment, and materials needed to complete the work. PART 2 -- DESCRIPTION OF WORK: 2.1 The Contractor shall perform the following work: a. Remove all flooring, base board, window casing and trim, stair treads and handrail, interior doors, and light fixtures. b. Install laminate flooring throughout second floor living room, second floor dining room, second floor kitchen, second floor hallways, first floor entry way and first floor hallways. STATION BAYFIELD housing rep to provide flooring specs. c. Supply and install new bi-fold doors for all closets. d. Inspect stair framing to ensure all current materials being used are still in working condition. Install new treads, risers, handrails, stair nosing, and any trim work required to ensure a neat and finished looking appearance is achieved. e. Repair any holes, cracks, or blemishes in all drywall to ensure a neat and finished looking appearance is achieved. All drywall that is worked on will be primed and made ready for a paint contractor to finish with a top-coat of paint. f. Supply and install 3 ¼" pre-primed casing for all doors and windows, 4 ¼"-5 ½" pre- primed base throughout the home. g. Supply and install lighting fixtures that are the same style and color as the ones located at 209 S. 11th Street home. h. Supply new vanity, top sink, and medicine cabinet for first floor bathroom. Replace vinyl flooring with ceramic tile. The style and color of tile to be used will be chosen by the STATION BAYFIELD housing rep. 2.2 The Contractor shall ensure that the worksite is left in a clean and orderly condition. Upon completion of all work, the STATION BAYFIELD housing rep will inspect all completed work for quality assurance. 2.3 The Contractor shall dispose of all waste materials off site at his own expense. 2.4 Any corrective actions that are needed for issues that are found during the completion of this work that will have a direct impact upon the habitability of the home00 should be brought to the attention of the STATION BAYFIELD housing rep and forwarded to the District 9 Owned Housing Maintenance Coordinator for approval. PART 3 - PROTECTION OF COAST GUARD PROPERTY: 3.1 Contractor shall bear all financial responsibility for any and all damages caused by contractor or an employee of the contractor while working on Coast Guard owned property. PART 4 - PRE-WORK MEETING/WORK HOURS: 4.1 STATION BAYFIELD housing rep and Contractor will jointly agree on a start Date and Time. 4.2 Normal work hours for Contractor will be 7:30 AM to 6:00 PM Monday through Friday. 4.3 Contractor may request extended work hours prior to start of work on a case by case basis. PART 5 - QUALITY ASSURANCE: 5.1 Upon completion of work, the STATION BAYFIELD housing rep will inspect work. Any deficiencies in workmanship and/or materials will be corrected prior to sign off of work completion or payment for services and materials provided. PART 6 - LIABILITY: 6.1 Under no circumstances will the Coast Guard assume liability for bodily injury to the Contractor or to his/her employees. 7. CONTRACTOR PERFORMANCE SPECIFICATIONS 7.1 All work shall be accomplished in a professional manner and in strict adherence to current manufacturer standards and recommendations. The Contractor, his/her employees, and subcontractors shall become familiar with and obey all station regulations. All personnel employed on the project shall keep within the limits of the work and avenues of ingress and egress, and shall not enter any other areas outside of the site of the work unless required to do so in the performance of their duties. The contractor shall provide drop cloths and/or hall runners to facilitate clean ingress and egress to the location in the homes where the work is to be done. The Contractor's equipment shall be conspicuously marked for identification. 7.2 There shall be NO SMOKING in any Coast Guard building or facility. 7.3 Contractor shall adhere to current codes, ordinances and OSHA safety regulations at all times. This includes the donning and proper wear of respiratory equipment and safety glasses that are OSHA rated and comply with current Coast Guard standards (COMDTINST 5100.47). 8. LIABLITY INSURANCE 8.1 It is the responsibility of the contractor to provide their own liability insurance for contractor and employees. In no case will the Coast Guard assume liability for bodily injury to the contractor and/or employees. 9. DEFECTIVE WORK 9.1 The Government reserves the right to reject departures from contract requirements, use of damaged or improperly installed items/materials, or latent defects. Any approvals provided prior to the start of work do not prejudice the Government's rights of rejecting any work found defective at Final Inspection and Acceptance. 10. UTILITIES AND OTHER FACILITIES 10.1 ELECTRICITY AND WATER: Electrical power, water and sanitary facilities are available on site. The Contractor will be permitted to utilize these utilities in performing the work, provided that the existing systems are not overloaded. The Contractor is responsible for installing and removing all connections to existing systems and shall ensure work and materials are in accordance with local codes. The use of the electricity shall be limited to tools that can be operated on 60 Hertz, single phase, 20 ampere, and 120-volt circuits. 10.2 TELEPHONE: Telephone services will not be available for use by the Contractor. 11.
