China for the Evaluation of Cytosponge screening for identifying the presence of esophageal squamous dysplasia in asymptomatic high-risk individuals. The Nutritional Epidemiology Branch (NEB), Division of Cancer Epidemiology and Genetics, NCI conducts intramural research in areas related to nutrition and cancer which are aimed at lowering human cancer risk. One focus area of NEB research is the etiology and early detection of esophageal cancer (EC), especially in high-risk populations. Over the past few years, researchers have developed an inexpensive encapsulated sponge, the Cytosponge, for non-endoscopically retrieving esophageal cell samples. This device is composed of a compressed plastic sponge enclosed in a gelatin capsule and attached to a string. The subject swallows the sponge into the stomach, the gelatin dissolves, and the expanded sponge is pulled up the esophagus by the string, collecting exfoliated and scraped superficial epithelial cells. The sponge is then rinsed in a liquid, the liquid is centrifuged, and the cell pellet is fixed and embedded in a paraffin block. The paraffin block is cut and slides are stained with HE or other stains and evaluated for atypical cells. This technique appears promising for population screening for Esophageal Squamous Dysplasia (ESD), but it has not previously been tested in a clinical screening situation in a high-risk population. Recently, researchers in China have performed a screening study in which they used the Cytosponge technique to try to distinguish dysplastic from non-dysplastic individuals during clinical endoscopic screening of asymptomatic adults in a high-risk population. The NCI would like to obtain raw data from this study, including the cytology results (patient acceptability and presence/absence of atypia), the endoscopic appearance of the mucosa after Lugol's staining, and the pathology of each biopsy in a spectrum of individuals, to more closely evaluate the The purpose of this study is to evaluate the ability of Cytosponge cytology to identify asymptomatic individuals with biopsy-proven esophageal squamous dysplasia. The contractor shall perform the following tasks Cytosponge cytology and Lugol's chromoendoscopy with biopsy of all unstained lesions on the following patients from a high risk population for Esophageal Squamous Cytosponge Cytology: •40 patients with a worst biopsy diagnosis of no dysplasia •10 patients with a worst biopsy diagnosis of mild squamous dysplasia •10 patients with a worst biopsy diagnosis of moderate squamous dysplasia •10 patients with a worst biopsy diagnosis of severe squamous dysplasia •10 patients with a worst biopsy diagnosis of invasive ESCC For each patient, the contractor shall deliver: •Demographic information (age, sex, county of residence) •Patient's evaluation of Cytosponge acceptability •Cytosponge cytology diagnosis •Endoscopic appearance of the esophagus after Lugol's staining, including the location and description of all unstained lesions (USLs) •Location, endoscopic appearance after Lugol's staining, and biopsy diagnosis of all biopsy sites The products herein are being procured in accordance with the simplified acquisition procedures as authorized by FAR Part 13.106-1 (b) (1). The North American Industry Classification System Code is 541380 and the business size standard in 500. Only one award will be made as a result of this solicitation. This will be awarded as a firm fixed price type contract. It has been determined that there is no opportunity to acquire green products or services under this procurement. Period of Performance: (3) months from the time of award. This is not a solicitation for competitive quotations. However, if any interested party, especially a small business, believes they can meet the above requirement, they may submit a proposal and or quote. All information furnished must be in writing and must contain sufficient detail to allow the NCI to determine if it can meet the above unique specifications described herein. The proposal must be received at the NCI Office of Acquisitions on or before 11:00 AM EST on May 23, 2014.
