- Drawing Schedule of Items (bid page) i. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. ii. The Request for Proposal number is AG-9A40-S-14-0016. iii. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-73 dated 04/29/2014. iv. This solicitation is being issued as a total small business set-aside. The NAICS Code is 531120. The small business size standard is $25.5 million. v. The Angeles National Forest requires the following items: Rental of one 12' x 60' office trailer. The rental will be for a one-year Base Period (estimated to be 07/01/2014 - 06/30/2015) along with up to three one-year option periods. The contract shall include all delivery and set-up fees, and optional tear-down and removal fees. Prices must be submitted for all items and option items. Prices quoted must be valid for a minimum of 60 days. vi. The Forest Service requires a 720 sq. ft. modular office building with dimensions approximately 12 ft. x 60 ft. The layout should provide four private offices, both men's and women's restrooms, and a large open shared office area for cubicles (as shown on the drawing). The building shall be new or in "like-new" condition, and must be covered by manufacturer's warranty. The building will be located at 29835 Valyermo Road, Valyermo, CA 93563. Delivery and set up shall include any necessary blocking and leveling, installation of standard ground anchors, and standard matching skirting around perimeter of building. Supporting structures to be included in the cost proposal include ADA accessible stairs and ramps. Trailer removal shall be priced as an option for termination of lease and will include dismantling and return freight charges and any other miscellaneous charges such as unblocking. The modular office building must comply with all applicable federal, state and local jurisdictional requirements and ordinances to include: • National electrical code • Americans with disabilities Act Guidelines • International Building CodeR • Required mechanical code • Uniform gas code • Required plumbing code • Uniform Federal Accessibility Standards (UFAS) • Occupational Safety and Health Act • NFPA standard for the installation of warm air heating and air conditioning vii. The office trailer shall be delivered, set-up, and ready for use by July 1, 2014. The delivery location is 29835 Valyermo Road, Valyermo, CA 93563. FOB Destination. viii. The provision at 52.212-1, Instructions to Offerors-Commercial, applies to this acquisition. The provision at 52.217-5, Evaluation of Options, is included as an addenda. ix. The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition, and the specific evaluation criteria are: • Delivery Schedule • Floor Plan • Past Performance • Price x. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. All offeror's must be registered at www.sam.gov. xi. The Clause 52.217-9, Option to Extend the Term of the Contract, is included as an addenda to the clause. xii.
