- I. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with no additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation number F14PS00526 is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. II. This procurement is for U.S. Fish and Wildlife Service (FWS) Southeast Louisiana National Wildlife Refuge (NWR). Requirement is as follows: line item 010: #610 Kentucky blue/gray Limestone; for delivery and stock pile to resurface Boy Scout Road located at/near the FWS Big Branch Marsh NWR; line item 020: #610 Kentucky blue/gray limestone; for delivery and stock pile for Gravel Pit Road on Bogue Chitto National Wildlife Refuge. III. This requirement is being procured in accordance with FAR Part 12, Acquisition of Commercial Items and an award will be made in accordance with FAR Part 13, Simplified Acquisition Procedures. This action is being solicited as a small business set-aside and only qualified offerors may submit a quote. The NAICS Code associated with this requisition is 212321 with a small business size standard of 500 employees. Buyer intends to issue award using a fixed priced purchase order or delivery order. Bids from Sellers unable to accept purchase orders or delivery orders will not be considered for award. IV. Offerors are cautioned that a warranted Contracting Officer is the only authorized Government Representative to make any contractual changes once an award is made. No oral statement of any person whomsoever shall, in any manner or degree modify or otherwise affect the terms of this Purchase Order. In the event the Contractor effects any such change at the direction of any person other than the Contracting Officer, the change shall be considered to have been made without authority and no adjustment shall be made in the Purchase Order price to cover any increase in costs incurred as a result thereof. The CO for this order is James_Promise@fws.gov. V. Government Representative (GR): Authorized Government Point of Contact for this order is Ginger Taylor at ginger_taylor@fws.gov. She shall not, however, have the authority to make any changes in the purchase order. VI. Description: (1.) Line Item 0010: #610 Kentucky blue/gray Limestone; for delivery and stock pile to resurface Boy Scout Road located at/near the FWS Big Branch Marsh NWR (2.) Line Item 0020: #610 Kentucky blue/gray limestone; for delivery and stock pile for Gravel Pit Road on Bogue Chitto National Wildlife Refuge. This work entails furnishing all necessary management, supervision, labor, materials, and equipment to supply a total of 535 tons of limestone. The contractor shall ensure total for all amounts supplied does not exceed the amount noted on the purchase order. VII. Deliveries will be at Gravel Pit Road on Bogue Chitto National Wildlife Refuge, West of Picayune, MS., as per attached map and to two other sites within Big Branch Marsh NWR, located in Lacombe, LA, C/O Southeast Louisiana National Wildlife Refuge at 61389 Hwy 434, Lacombe, La 70445 as identified and shown on the attached map. 1.Big Branch Marsh NWR, Boy Scout Road: The distance and directions to the location from exit #74 on Interstate 12 (I-12) are as follows. Exit I-12 at exit #74 and turn south onto highway 434. Travel 2.6 miles on highway 434 and take the second exit through the turnabout. Head East for 2.0 miles and turn right onto Transmitter Rd. Head two miles and dead end at Bayou Paquet Rd. Turn Right (West) and travel 1 mile and turn left onto Boy Scout Rd. 150 tons of limestone will be delivered at a designated spot by refuge personnel (Delivery Point 1) and another 210 tons at Delivery Point 2 as designated by refuge personnel. 2.Bogue Chitto NWR, Gravel Pit Road: The distance and directions to Gravel Pit Rd from Slidell (Interstate 10) are as follows. From interstate 10 head north and merge onto Interstate 59 toward Picayune, Mississippi. From Interstate 10 to Picayune Mississippi is 18 miles. Exit Hwy 43N in Picayune Mississippi. Turn West (left) onto Hwy 43N for 10 miles before turning left onto Pine Grove Rd for 1.5 miles. Then turn right onto Dumas Wise Rd. for another 3.34 miles. Turn left onto Luther Wise Rd. for 0.1 mile and turn right onto an unlabeled gravel road and refuge gate (called Gravel Pit Rd). 175 tons of limestone will be delivered to this site (Delivery Point 3) as per refuge personnel on the day of delivery. Please contact the technical point of contact (Danny Beaux, 985-882-2000, danny_beaux@fws.gov) for exact location specifications. At minimum of 200 tons would be expected daily. There are county road weight limit restrictions coming into this site the vendor needs to be aware of. The vendor would need to verify with the county road commissioner if a bound is required to haul on this road. Once delivery begins, the Contractor shall deliver at a rate of not less than 13 truckloads per day during regular working hours. Hours are 7:30 A.M. to 4:00 P.M. Delivery will not be made on weekends or when conditions are so wet that delivery will damage existing roads. The delivery to each of the refuges will be made on separate days to make allowance for refuge personnel to travel to the site for delivery assistance. All three delivery locations require gates to be opened by refuge personnel for the days delivery. Identified delivery location will be at designated through site manager. All deliveries will occur within an active jobsite or staging area that will accommodate highway compliant vehicles, required delivery trucks, etc. Drivers shall furnish certified scale tickets with the actual cost by tons delivered to FWS/government personnel upon delivery of each load to the delivery site. Required commencement date for delivery is on or about June 18, 2014 or sooner. Last day for delivery is August 1, 2014. For inquires on delivery site details, exact locations and specifications, please reference attached Roadway/Gravel Surface Material sheet and map or contact the technical point of contact, Daniel Breaux. NOTE: It is the Contractors responsibility to visit the site prior to bidding and know existing delivery and dumping conditions. VIII. Local conditions (bridge restrictions, weather conditions, water levels, road flooding, etc.) that may affect delivery: None. Contractor shall coordinate deliveries (including hours of delivery) with GR 48 hours in advance. IX. System for Award Management (SAM) Registration - A prospective awardee shall be registered in the SAM database prior to award. Information on registration may be obtained via the Internet at: https://www.sam.gov/portal/public/SAM/#1 or by calling 866-606-8220. Failure to maintain a current/active account so will render your offer non-responsive. X. RFQ Due Date: Telephone quotes will NOT be accepted. All quotes must be received at this office no later than June 4, 2014, 11:00 AM Local Eastern Time. ONLY EMAILED QUOTES WILL BE ACCEPTED. Email to constance_fields@fws.gov. Attention to: Constance Fields. Please reference/subject: solicitation number and Gravel Solicitation Southeast Louisiana Refuge. Quotes should be attached document that is either Microsoft Word 97 compatible or PDF. The anticipated award date is on or about June 11, 2014. Please do not request award status prior to this date. All questions must be submitted no later than May 23, 2014 by 11:00 AM local eastern standard time. XII. All interested Contractors shall provide a quote as follows: (1) SF 1449 is acceptable for submitting quote. Parties responding to this solicitation may also submit their offer in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc.), but at the minimum include the following information: (2) Line Item 0010: Crushed Limestone for Big Branch Marsh NWR, Boy Scout Road; Price per Ton $________; Total Price $_______; Line Item 0020: Crushed Limestone Bogue Chitto NWR, Gravel Pit Road $________; Price per Ton $________; Total Price $_______; Line 0010 and Line 0020; Grand Total___________________________. (3) Company's name, point of contact, address, phone number, fax number, email address, solicitation number, item Description/Specification (Descriptive literature if applicable), total price (itemize price final total price),and approximate daily delivery schedule ( include number of trucks, amount to be delivered, and total number of days to complete requirement). Price(s) should be for FOB Destination. (4) A list of three (3) most recent past performance references for a similar purchase including name/point of contact, address, phone number, and email address; (5) Statement confirming registration in SAM (www.sam.gov). Statement confirming Representation and Certification completion within SAM. Provide account expiration date, cage code, DUNs number, and taxpayer ID number; (6) Statement confirming that you will accept electronic payment through IPP. (www.ipp.gov) NOTE: Any quotes not received by the due date and time specified in this synopsis/solicitation will be rejected. Additionally, any incomplete quote not containing the required information as stated in this solicitation may also be rejected. NOTE: The final quoted price(s), both total and individual, MUST include all applicable fees, gratuities, commissions, service charges, etc. FEDERAL GOVERNMENT IS TAX EXEMPT.
