Description
Performance Work Statement This is a posting for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z4544120AQ01-PregnancyFitnessCenter. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-72 effective 29 Apr 2014, Defense DPN 20140506 effective 06 May 14, and AFAC 2014-0421 effective 21 Apr 2014; the North American Industry Classification System (NAICS) code is 611620. The business size standard is $7,000,000.00. The Federal Supply Class (FSC) is Q518. The Standard Industrial Classification (SIC) is 7999. The requirements are as follows: CLIN 0001 - Pregnancy and Postpartum Fitness Instructor at the Health and Wellness Center. Fitness Instructor will have to perform in accordance with the Performance Work Statement. Specific Tasks of the Contractor Include: a) Provides classes at the Travis Fitness Center/HAWC or other area on base that is conducive to fitness b) Classes are conducted in on hour time increments, Monday-Sunday, between the hours of 7 AM and 9 PM. Classes are offered on days and at times that meet the needs of the participants. At least one class per week will be taught and no more than eleven classes per week will be taught. Class schedules are flexible to accommodate the base population's needs c) Contacts a qualified instructor to substitute in the event that a previously scheduled class cannot be taught due to illness, injury or vacation. Contacts the HAWC as soon as possible if a class cannot be covered d) Records each class instructed and maintains neat and accurate daily attendance records in the designated binder e) Submits documentation for payment no later than the 1st workday following the end of the month f) Provides appropriate music for the classes. g) Takes responsibility for set p and take down of class equipment h) Ensures all equipment is properly stored at the completion of class. i) Provides courteous, safe, and quality service to patrons j) Begins and ends classes on time. All classes should end five minutes before the hour to allow for a smooth transition between classes k) Has current certification from a nationally recognized certifying organization as well as a current CPR card. Contracts shall not exceed CPR or certification expiration date. Copies of all certifications must be submitted to the HAWC Director before a contract can be renewed. Quantity: 572 classes each Please provide separate prices for 01 October 2014 - 30 September 2015 and each of the following option years. CLIN 1001- Option Year 1: 1 Oct 2015 - 30 Sep 2016 ______________________ CLIN 2001- Option Year 2: 1 Oct 2016 - 30 Sep 2017 ______________________ CLIN 3001- Option Year 3: 1 Oct 2017 - 30 Sep 2018 ______________________ CLIN 4001- Option Year 4: 1 Oct 2018 - 30 Sep 2019 ______________________ This requirement will be 100% Disabled Veteran Owned Small Business Set Aside. Quotes must be in accordance with the attached Performance Work Statement. All other contractor's terms and conditions do not apply. Award shall be made in the aggregate, all or none. (End of clause) 5352.201-9101 Ombudsman OMBUDSMAN (APR 2010) (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source sele ction official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication of protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee -employer actions, contests of OMB Circular A-76 competition performance decisions).
