- Please see attached forms referenced in RFQ Please see attached RFQ for Combined Synopsis/Solicitation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6 in combination with FAR Subpart 13.5 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 60-14MR-2295 and it is issued as a Request for Quotes (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-72. The associated North American Industrial Classification System (NAICS) code for this procurement is 621111, Offices of Physicians (except Mental Health Specialists). Only qualified Offerors will be considered. Contract line items are outlined within the attached document labeled Physicals IDIQ RFQ. Period of Performance: The base period of performance will be 12 months from the date the contract is awarded with one option period of 12 months to allow for all options to be exercised and performed within that time period depending on scheduling and availability of funds. Offerors must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with your quote or be fully registered in the System for Award Management (SAM). Quotes must be received by Aleisha Moore, Purchasing Agent, no later than 12:00 PM (EST) on May 27, 2014. Quotes will be accepted by e-mail at moore.aleisha@dol.gov. Electronic quotes must be submitted in a format compatible with Microsoft Office or Adobe. Quotes must be valid for a period of 45 days and include enough information for the Government to make a determination that the Contractor can responsibly deliver the required products and services. All questions on this requirement must be submitted in writing via e-mail to the Purchasing Agent by May 20, 2014 at 12:00 pm (EST). Any questions for which answers can be provided may be conveyed to prospective Offerors by amendment to this RFQ on or around May 22, 2014, without reference as to the originating source. Phone calls will not be accepted.
