Description
This is a combined solicitation/synopsis for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, N00259-14-T-0168 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 811219; Size: $19.0M. STATEMENT OF WORK (SOW) NAVAL HEALTH CLINIC HAWAII DENTAL WASHERS 22 April 2014 1.0 BACKGROUND: Commanding Officer Naval Health Center Hawaii (NHCH) is the provider of health services for over 18,000 active duty naval personnel and 37,000 family members based on Oahu, Hawaii. NHCH is comprised of two large clinics, Branch Medical Clinic Makalapa at Pearl Harbor and Branch Medical Clinic Kaneohe Bay at Marine Corps Base Hawaii, Kaneohe Bay, and smaller clinics Branch Medical Clinic Camp Smith, Branch Medical Clinic Wahiawa, and Branch Medical Clinic Barking Sands, Kauai and SMART Physical Therapy clinics at Makalapa and Kaneohe Bay. 2.0 SCOPE/OBJECTIVE: Contractor will provide full service and preventative maintenance on existing equipment. 2.1 EQUIPMENT: Model 533HC Sterilization, 21" Sm SN: 10H00196 ECN: 015646 Model GTS-30A Boiler SN: 41231 Model 8666, Clean/Disnf, Vaxjo SN: W50038691 ECN: 017049 Model 533HC Sterilization, 21" Sm SN: 06B06631 ECN: 014809 Model CAS30C Generator/Boiler SN: GC-32437-V06 Model HS-2400FT Watts Reverse Osmosis Modernization SN: 102400416 ECN: 015877 3.0 PERFORMANCE REQUIREMENTS: a. The Contractor shall perform all necessary tasks described in this SOW. This includes: b. Total Care Preventative Maintenance Plan, Quarterly. c. Inspection, troubleshooting. d. Repair, replace with all service parts needed. e. Contractor shall conduct quarterly preventive maintenance inspections and service maintenance, over a one year period, on the sterilization equipment located at the Makalapa Dental Clinic, BLDG 1407B. This equipment will include: the two GETINGE steam sterilizers, one GETINGE instrument washer, a reverse osmosis machine and two boilers servicing this equipment. The preventive maintenance and service maintenance visits include: on-site repair service and parts. 3.1 Technical support number to call for Getinge Customer Service @ (800) 950-9912 Option 1. Normal hours of operation are (8 AM-5 PM) Monday-Friday. On site services will be performed at no charge within 24 hours of notification, Monday to Friday, during normal business hours (8 AM-5 PM). Outside these hours, on weekends and holidays troubleshooting is offered over the phone, through the On-Call representative. After hours - Work performed after 5PM through 8AM, Monday through Saturday, (excluding national holidays) will be billed at 1.5 times the preferred labor rate at a minimum of 4 hours, portal to portal. Emergency after hours - Emergency after hours work performed on a Sunday or national holiday will be billed at 2 times the preferred labor rate at a minimum of 4 hours portal to portal. 3.2 The Contractor shall perform all tasks described by this PWS in accordance with applicable OSHA, Joint Commission of Accreditation of Healthcare Organization, state and federal requirements. All costs associated with proper use of personal safety equipment by Contractor personnel shall be the responsibility of the Contractor. 3.3 All work shall be coordinated with the BIOMED Technical Point of Contact (TPOC) to minimize disruptions to clinic operations at all locations. Perform tasks as outlined in PWS. Paragraph 3.0 100% delivery and compliance. 4.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES 4.1 GENERAL: See EQUIPMENT Section 2.2. Naval Health Clinic Hawaii currently owns this GETINGE equipment. Reverse Osmosis filters will be purchased and provided by Naval Health Clinic Hawaii. 5.0 UTILITIES: The contractor is to use the electrical provided by Navy Health Clinic Hawaii. 6.0 CONTRACTOR FURNISHED EQUIPMENT/MATERIAL/SERVICES: The contractor will provide the following equipment/material in performance of this contract: GETINGE SIGNATURE SERVICE shall provide parts and labor for equipment inspections, adjustment, lubrication, installation of replacement parts and testing during scheduled preventive maintenance. Parts and Labor for repair calls between preventive maintenance inspections and service maintenance which are the result of equipment failure is also included. Any needed replacement parts identified during the preventive maintenance inspections and service maintenance will be ordered and installed upon arrival. Replacement parts shall be new. 7.0 CONTRACTOR QUALIFICATIONS REQUIREMENTS 7.1 The Contractor shall be the qualified service maintenance provider and installer. 7.2 In accordance with current Department of Defense (DOD) and Department of Homeland Security (DHS) requirements, performance of all work under this contract is restricted to US citizens and US nationals only. The Contractor shall provide proof of this requirement upon request of the Contracting office. 7.3 Security Requirements: This contract does not require a security clearance level and clearance granted by the Defense Security Service (DSS). 8.0 PERIOD OF PERFORMANCE: 01 JUN 2014 - 31 MAY 2015 Maintenance shall be on a quarterly basis: Preventative Maintenance Schedule: 1st - June 3, 2014 2nd - September 2, 2014 3rd - December 2, 2014 4th - March 3, 2015 LOCATION: BRANCH HEALTH CLINIC, MAKALAPA DENTAL DEPARTMENT BLDG 1407B MAKALAPA DRIVE JOINT BASE PEARL HARBOR HICKAM, HI. 96860 (808) 471-1880 (x3372) 8.1 During all hours of operation, all Contractor personnel shall wear pictured identification cards that identify the employee's name and the Contractor's business name, plainly visible in the upper chest area. Any special Government badges provided shall be worn and visible. All Contractor employees shall present a neat and clean appearance in accordance with generally accepted standards. The work leader shall be able to proficiently speak, read, write and understand English. 8.2 The Contractor shall not employ Government personnel (military or civilian) for performance of any work described in this SOW. 8.3 The Government reserves the right to direct the removal of any of the Contractor's personnel for misconduct or security reasons. Removal of the Contractor's employee(s) for reasons stated above does not relieve the Contractor from the PWS performance requirements. 8.4 Contractors shall not provide personal services to NHCH under this PWS. If contract employees are directed by any government employee at any time to perform services not covered under this contract, the Contractor shall immediately notify the Commanding Officer (CO). 8.5 Contractor personnel shall not manage, supervise, direct, or task US military, DOD civilians or other personnel not associated with the Contractor while performing the tasks within this PWS. 8.6 Contractor personnel shall not make any commitment which commits the expenditure of US Government resources. 8.7 The Contractor performing services shall be required to comply with all JBPHH and NHCH rules, regulations and training that is applicable to conduct, safety, security, and procedures governing site entry and exit. 8.8 Contractor personnel shall not establish an employee-employer relationship with NHCH under this PWS-Performance Work Statement. 9.0 POINTS OF CONTACT: The phone number for BIOMED (TPOC) coordination is (808) 473-1880 (x3437). 9.1 Technical POC: Frances Baxter, (808) 474-0618, frances.baxter@med.navy.mil or Raoul Allen (808) 471-2594, raoul.allen@med.navy.mil.
