CLASSIFICATION CODE: Q - Medical Services; (Q201 - Medical - General Health Care) NAICS CODE: 621 - Ambulatory Health Care Services; 621498 - All Other Outpatient Care Centers Network-9 Contracting Office on the behalf of the Memphis VA Medical Center has developed this Request for Information (RFI)/Sources Sought Notice to solicit market research information from the healthcare industry to assist the Government with analysis of a Government requirement and provide commercial insight on performance-based solutions, best practices, possible contractual arrangements, and potential issues. THIS IS A REQUEST FOR INFORMATION ONLY. It is not a Request for Proposal, a Request for Quotation, an Invitation for Bids, a Solicitation, or an indication that Network-9 Contracting Office (NCO-9) will contract for the items contained in the RFI / Sources Sought Notice. This RFI/Sources Sought Notice is part of a Government market research effort to determine the scope of industry capabilities and interest, in accordance with Federal Acquisition Regulation (FAR) Part 15.201(e), response to this RFI / Sources Sought Notice are not offers and cannot be accepted by the Government to form a binding contract. Responses to this RFI / Sources Sought Notice are strictly voluntary and are consider as information only. The Government will not pay respondents for information provided in response to this RFI / Sources Sought Notice. Responses to this RFI / Sources Sought Notice will not be returned and respondents will not be notified of the result of the review. If a solicitation is issued, it will be announced on the Federal Business Opportunities Website http://www.fbo.gov at a later date, and all interested parties must respond to that solicitation announcement separately from any response to this announcement. This RFI / Sources Sought Notice does not restrict the Government's acquisition on a future solicitation. Network-9 Contracting Office (NCO-9) and Memphis VA Medical Center is conducting market research for a Contractor(s) to provide Community Based Outreach Clinic (CBOC) to provide Primary Health Care (PHC) and Mental Health Care (MHC) for approximately 1400 Veteran beneficiaries of the Department of Veterans Affairs (VA) and the Memphis VA Medical Center, 1030 Jefferson Avenue, Memphis, TN 38104-2127. GENERAL: Services Required - a.The Memphis (TN) Veterans Affairs Medical Center (Memphis VAMC) "the parent facility" requires CBOC services including the provision of Primary Health (PH), Comprehensive Women's Health Care, and Mental Health (MH) services in a private hospital, office or clinic environment to Veterans, primarily residing in the following Tennessee counties: Crockett, Dyer, Gibson, Lake, Lauderdale, and Obion Counties. The current contract for services is expiring and the Memphis VAMC has identified a need to continue receiving these services for Veterans residing within the county coverage area for this CBOC. b.The CBOC is established to improve access to veterans in rural areas to primary and specialty care. The CBOC will be located in an area that will greatly reduce the veteran's drive time. The CBOCs highest priority is to improve access to care for high priority enrolled veterans. Improved quality of care will reduce waiting times for follow-up care, in addition to improve waiting times at the parent hospital. The CBOC will be serving patients in a medically underserved manpower and a Primary Care Health Professional Shortage Areas. c.The Contractor shall provide a primary CBOC facility in or around the city of Savanna, Tennessee and within 5- to 10-miles of a local hospital providing emergency room services. d.Description of Services to be provided: 1)Primary care and preventive health services will be provided through the contract, with basic laboratory and radiology diagnostic examinations to be factored into the agreement. Other routine testing, such as EKG's and colorectal cancer screening will be performed by contract staff at the facility. 2)The CBOC will be aligned under the CBOC Coordinator, who has direct line authority over operations and monitoring of the contract. 3)Patients presenting to the CBOC requiring emergency care will be transferred to the nearest appropriate healthcare facility as soon as possible. Hours of operation and directions on how to seek care through telephone care for routine and non-emergent issues, as well as instructions to report to the nearest emergency room for potentially emergent or life-threatening situations, will be clearly posted and provided to patients who call the clinic during other than operating hours. 4)Patients requiring specialty care will be referred to Memphis VAMC, unless their condition requires immediate attention, in which case they will be referred to the closest appropriate community resource. Tele-health will be utilized for some specialty care. 5)Mental health services will be provided through a contract arrangement with either the contractor for primary care or through an additional contract with a properly trained and prepared mental health professional. Services provided will include mental health screening and evaluation, medication management, counseling and referrals for specialized care as needed. Those patients requiring more extensive consultation or treatment will be referred to the Memphis medical center, or to the community if the situation warrants immediate action. 6)Specialty care will be provide through referral to Memphis VAMC, dependent upon the specialty involved. Patients requiring treatment prior to its availability at Memphis VAMC will be referred to a community resource if necessary. 7)Quality of Care monitoring will include requirements that the contractor conform to VA quality of care requirements, performance measures, and other operational and regulatory practices.
