Description
Table - HIV POST EXPOSURE PROPHYLAXIS KIT The Naval Medical Logistics Command (NMLC) in support of the US Naval Fleet is seeking information about the availability of industry to provide HIV POST EXPOSURE PROPHYLAXIS (PEP) KITS. THIS IS A REQUEST FOR INFORMATION (RFI) ONLY issued pursuant to Federal Acquisition Regulation (FAR) 15.201(e). This RFI is issued solely for planning purposes - it does not constitute a Request for Proposal (RFP) or a promise to issue an RFP in the future. This request for information does not commit the Government to contract for any supply or service whatsoever. Further, NMLC is not at this time seeking proposals and will not accept unsolicited proposals. No award is intended as a result of this request. Responders are advised that the U.S. Government will not pay for any information or administrative costs incurred in response to this RFI. All costs associated with responding to this RFI will be solely at the interested party's expense. Not responding to this RFI does not preclude participation in any future RFP if one is issue, nor does responding give any preference. Interested parties should review the attached documents and reply no later than May 27, 2014 NMLC is requesting information on product availability, cost and lead time of kits below. Shelf-Life components shall have 85% of shelf life available at time of delivery. Each PEP Kit will contain items in the chart below. Total requirement of 216 Kits will be shipped to approximately 5 separate locations in the Continental United States. Each kits will be separately marked and distributed as one unit. Please provide the following information: 1. Company name, address, CAGE number and POC contact information 2. What is your business size and/or socioeconomic category? 3. Do you offer any substitutions for the above products? 4. Do you offer other units of issue than indicated above? 5. What are the prices of the items above? Are there any quantity discounts available? 6. Can you provide the above items in a kit? 7. What is the suggested NAICS code? 8. What is the ARO of 216 kits? 9. Do you have a Federal Supply Schedule? If so, what is the contract number? 10. Any comments on current market conditions or obstacles to providing the requirements above. See attached table. Responses and information should be sent to tameka.davis@med.navy.mil by 1pm on 27 May 2014. This is a request for information only. No award will be made as a result of this RFI.
