Amendment 3: Deletes Line Item 1. Line Item 2 replaces Line Item 1, to change Ship To/Place of Contract Performance address to: Tetra Tech, 34 West School Street, Bonne Terre, MO 63628 Also, answers vendor questions: 1) Question: The RFQ mentions "vendor must provide storage in St. Francois County". Is there a problem with our distributor keeping in their warehouse and deliver in quantities of 50, as needed? Answer: This will not be a problem. The vacuums and filters can go to the Tetra Tech Field Office for storage of 50 per delivery. Per the Statement of Work for the START contract, the START contractor shall store, track, and distribute HEPA vacuums to residents. See above Place of Contract Performance on this amendment. 2) Question: The vacuums mentioned in the RFQ, will these be ordered all at once or 50 at a time? If all at once, great. However, if not, then please supply a timeline as to when each lot will be purchased. Answer: We can order 50/month for the first 2 month then we will order 50 every 3 months after until we reach a total of 250. 3) Question: I'd like to place a bid on this solicitation however I do not find a Standard Form 18 or General Business Questions. Could you please email those to me? Answer: You may find the full solicitation on https://www.fedconnect.net/FedConnect/Default.htm Please select the Reference Number as RFQ-KS-14-00004. There is where you will find the Standard Form 18 and General Business Questions. See amendment 2. 4) Question: All of my contractor information regarding Representations and Certifications are on the sam.gov website. Will that suffice? Answer: As long as the information found about your company on Representations and Certifications is accurate, then that information can be utilized. This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number RFQ-KS-14-00004 is applicable and is issued as a request for quotation. The solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. The requirement is "small business set-aside" with a NAICS Code of 335210 (Small Electrical Applicance Manufacturing) and a size standard is 750 employees. A Wholesale Trade or Retail Trade business concern submitting an offer or a quote on a supply acquisition is categorized as a non-manufacturer and deemed small if it has 750 or fewer employees and meets the requirements of 13 CFR 121.406. Request for quote is being requested for the equipment listed bel ow to fulfill the Government's requirement: 1. Upright HEPA Vacuums - Quantity of 250 each 2. Corresponding HEPA Filters - Quantity of 250 each. The filters will be ordered/purchased in lots of 50; model specs of the vacuums include electrostatic filter, HEPA vacuum bag, 12 amps of power, on-board tools, 30-foot cord, metal components throughout. Suggested model Riccar Vibrance Deluxe. Vacuums and filters will be purchased in lots of 50. Products must meet specifications. If proposing equal, please document your quote with evidence of this fact and submit an alternative model that is equal to the requirements specified. You must include any available descriptive literature. Please specify the brand that will be used. The Government intends to make award on an "all or none" basis. The selection resulting from this request for quotation will be made on the basis of the lowest priced, technically acceptable quote from a responsible quoter with the ability to furnish all of the products within the required delivery schedule (30 days ARO). It is the sole responsibility of the contractor to demonstrate its ability to provide the products specified above. The Contracting Officer is not responsible for locating or obtaining any information not identified in an offeror's quote. Once the Contracting Officer has decided to whom an order will be issued, a unilateral order will be issued using an Optional Form 347. A purchase order will be in existence once the Contracting Officer has signed the order. Wherever the words offer, proposal, offerors, or similar terms are used in this solicitation or in any applicable provision, they shall be read to mean quote, quotation, quoter, vendor or similar corresponding term to reflect that this solicitation is a Request for Quotation not a Request for Proposal or an Invitation for bid. Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The Contracting Officer will evaluate quotes on the basis of information furnished by the quoter. The Contracting Officer is not responsible for locating or obtaining any information not identified in the quote. Telephonic inquiries will not be accepted. Quotes must be emailed or mailed and are due on May 23, 2014 at 12:00 PM Central Daylight Time (CDT) along with the completed representations and certifications, providing the business size, e-mail address, Dunn and Bradstreet number, CAGE code and Federal Tax ID number. In addition, quoters must be registered in the System for Award Management (SAM) database at www.sam.gov to be considered for selection. Quoters are required to submit completed copies of the provisions at FAR 52.212-3 and its ALT I, Offeror Representations and Certifications-Commercial Items, with their quotes. FAR clause 52.212-4, Contract Terms and Conditions Required to Implement Statutes or Executive Orders, Commercial Items, applies to this acquisition. INSTRUCTIONS Offers shall submit by email, fax, or mail: At a minimum, responsible sources shall provide the following: a price proposal using the attached Standard Form 18, completed Representations and Certifications, and complete the attached General Business Questions attachment. The submitted offerer shall also provide evidence that the items quoted meet all of the minimum requirements listed in the specifications list attached to this solicitation.
