This announcement constitutes the only solicitation and a written solicitation will not be issued. Proposals are being requested for Request for Proposal (RFQ), VA528-14-Q-0311 for the following product: 4 EA. RETRIEVE BONE PLUG INSTRUMENT Globus Item #691.020 4 EA. HAMMER,CANNULATED LOCAL STOCK Globus Item# 691.024 4 EA. GRAFT REMOVER Globus Item691.025 4 EA. BONE GRAFT HARVEST CASE Globus Item #991.005 2 EA. REGENERATIVE BIOMATERIALS Globus Item #991.006 These items are globus. Sellers submitting a qualified equal item needs to follow the instructions in FAR 52.211-6 Brand Name or Equal. As prescribed in 11.107(a), insert the following provision: BRAND NAME OR EQUAL (AUG 1999) (a) If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation. (b) To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must- (1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by- (i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications. (c) The Contracting Officer will evaluate "equal" products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the solicitation. Solicitation VA528-14-Q-0311 is issued by Stratton V.A. Medical Center 113 Holland Ave, Albany, NY 12180 who anticipates not conducting discussions with offerors submitting quotations under the subject solicitation and reserves the right to make contract award based on the initial offer submitted. Requirements may be added by future amendments to this solicitation. The Government contemplates award of a Firm Fixed Price contract resulting from this solicitation. SYNOPSIS/SOLICITATION Proposals must be received no later than 4:00 pm, EST on May 28, 2014. Offers may be submitted via email to Anthony Murray, Anthony.Murray2@va.gov. When submitting offers, please note the solicitation number VA528-14-Q-0311 in subject line of the email. Please note that the offeror assumes all risk for any delay in the transmission of their proposals. Any offers received after the 4:00 PM EST deadline will not be considered. All offerors are encouraged to review FAR 52.212-1, INSTRUCTION TO OFFERORS-COMMERCIAL ITEMS and Internal Instruction, INT L2.11-4, E-MAIL QUOTATIONS and to submit their proposals early.
