- SF-18 FORM COMBINED SYNOPSIS/SOLICITATION JANITORIAL SERVICES AT Las Vegas WFO Nevada (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice and in accordance with the simplified acquisition procedures authorized in FAR Part 13. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number NWWT93861402187DLZ. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-73. (IV) This solicitation is being issued as a HubZone Set-Aside The associated NAICS code is 561720. The small business size standard is $16.5 million. (V) This combined solicitation/synopsis is for purchase of the following commercial services: BASE YEAR: Line Item 0001 - Base Year; Services, non-personal, to provide all labor, equipment and materials necessary for Janitorial Services at Las Vegas, Nevada, WFO in accordance with the Statement of Work for the period of twelve months. OPTION YEAR 1: Line Item 1001 - Option Year 1; Services, non-personal, to provide all labor, equipment and necessary for Janitorial Services at Las Vegas, Nevada,. WFO in accordance with the Statement of Work for the period of twelve months. OPTION YEAR 2: Line Item 2001 - Option Year 2; Services, non-personal, to provide all labor, equipment and necessary for Janitorial Services at Las Vegas, Nevada, WFO in accordance with the Statement of Work for the period of twelve months. OPTION YEAR 3: Line Item 3001 - Option Year 3; Services, non-personal, to provide all labor, equipment and necessary for Janitorial Services at Las Vegas, Nevada, WFO in accordance with the Statement of Work for the period of twelve months. OPTION YEAR 4: Line Item 4001 - Option Year 4; Services, non-personal, to provide all labor, equipment and necessary for Janitorial Services at Las Vegas, Nevada, WFO in accordance with the Statement of Work for the period of twelve months. (VI) Description of requirements is as follows: See attached Statement of Work which applies to Base Year and all Option Years 1 - 4,and Department of Labor Wage Rates: WD #2005-2331 Revision No. 13 dated 6/19/2013 which can be found on: www.wdol.gov. (VII) Period of performance shall be at WFO Las Vegas, Nevada, 7851 Dean Martin Drive, Las Vegas Nevada 89139-9744. Base Year for a twelve month period 7/01/2014 through 6/30/2015. Option Period I for a twelve month period 7/01/2015 through 6/30/2016 Option Period II for a twelve month period 7/01/2016 through 6/30/2017 Option Period III for a twelve month period 7/01/2017 through 6/30/2018 Option Period IV for a twelve month period 7/01/2018 through 6/30/2019 (VIII) FAR 52.212-1, Instructions to Offerors -- Commercial Items (Apr 2014), applies to this acquisition. NOTICE TO OFFERORS: Instructions for submitting quotations under this request for quote must be followed. Failure to provide all information to aid in the evaluation may be considered non-responsive. Offers that are non-responsive may be excluded from further evaluation and rejected without further notification to the offeror. 1. Quotes shall be fully executed and returned on the Standard Form (SF) 18 and any acknowledgements of solicitation amendments on the SF 30. 2. Submit quotations to the office specified in this solicitation at or before the time specified in the solicitation. At a minimum the contractor shall provide the following information: a) Point of contact name, telephone and E-mail address. b) DUNS Number 4. Provide all evaluation criteria in accordance with 52.212-2 in this package. 1352.215-72 Inquiries (Apr 2010) Offerors must submit all questions concerning this solicitation in writing to Donald.L.Schartz@noaa.gov. Questions should be received no later than 3:00 P.M. MST, May 28, 2014. Any responses to questions will be made in writing, without identification of the questioner, and will be included in an amendment to the solicitation. Even if provided in other form, only the question responses included in the amendment to the solicitation will govern performance of the contract. Vendors are urged and expected to inspect the site where services are to be performed and to satisfy themselves regarding all general and local conditions that may affect the cost of contract performance, to the extent that the information is reasonably obtainable. In no event shall failure to inspect the site constitute grounds for a claim after contract award. (IX) FAR 52.212-2, Evaluation -Commercial Items (Jan 1999), applies to this acquisition. Offers will be evaluated based on price and the factors set forth in paragraph (a), and award will be made to the firm offering the best value to the Government. Paragraph (a) is hereby completed as follows: Evaluation will be based on 1) Past Performance - quote shall include two references including the phone number, full address and email (if available) and contractor's past performance will be based on responsiveness, quality, and customer services and the Government will use its discretion to determine the sources of past performance information used in the evaluation, and the information may be obtained from references provided by the offeror, the agency's knowledge of contractor performance, other government agencies or commercial entities, or past performance databases. 2) Price. The Government intends to award a firm-fixed price purchase order on an all or none basis with payment terms of Net 30. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. 1) Capability to meet the requirements of the Statement of Work. • Vendor must be able to support this type janitorial service. 2) Price. • Based on Statement of Work • Rental - if applicable • Licenses - if applicable • Any other associated fees - if applicable The Government intends to award a firm-fixed price purchase order on an all or none basis. (X) FAR 52.212-3 Offeror Representations and Certifications - Commercial Items (Aug 2013) An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically via https://www.acquisition.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (o) of this provision. (a) Definitions. As used in this provision- "Economically disadvantaged women-owned small business (EDWOSB) concern" means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. It automatically qualifies as a women-owned small business eligible under the WOSB Program. "Forced or indentured child labor" means all work or service- (1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or (2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties. "Inverted domestic corporation", as used in this section, means a foreign incorporated entity which is treated as an inverted domestic corporation under 6 U.S.C. 395(b), i.e., a corporation that used to be incorporated in the United States, or used to be a partnership in the United States, but now is incorporated in a foreign country, or is a subsidiary whose parent corporation is incorporated in a foreign country, that meets the criteria specified in 6 U.S.C. 395(b), applied in accordance with the rules and definitions of 6 U.S.C. 395(c). An inverted domestic corporation as herein defined does not meet the definition of an inverted domestic corporation as defined by the Internal Revenue Code at 26 U.S.C. 7874. "Manufactured end product" means any end product in Federal Supply Classes (FSC) 1000-9999, except- (1) FSC 5510, Lumber and Related Basic Wood Materials; (2) Federal Supply Group (FSG) 87, Agricultural Supplies; (3) FSG 88, Live Animals; (4) FSG 89, Food and Related Consumables; (5) FSC 9410, Crude Grades of Plant Materials; (6) FSC 9430, Miscellaneous Crude Animal Products, Inedible; (7) FSC 9440, Miscellaneous Crude Agricultural and Forestry Products; (8) FSC 9610, Ores; (9) FSC 9620, Minerals, Natural and Synthetic; and (10) FSC 9630, Additive Metal Materials. "Place of manufacture" means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture. "Restricted business operations" means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate- (1) Are conducted under contract directly and exclusively with the regional government of southern Sudan; (2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization; (3) Consist of providing goods or services to marginalized populations of Sudan; (4) Consist of providing goods or services to an internationally recognized peacekeeping force or humanitarian organization; (5) Consist of providing goods or services that are used only to promote health or education; or (6) Have been voluntarily suspended. "Sensitive technology"- (1) Means hardware, software, telecommunications equipment, or any other technology that is to be used specifically- (i) To restrict the free flow of unbiased information in Iran; or (ii) To disrupt, monitor, or otherwise restrict speech of the people of Iran; and (2) Does not include information or informational materials the export of which the President does not have the authority to regulate or prohibit pursuant to section 203(b)(3) of the International Emergency Economic Powers Act (50 U.S.C. 1702(b)(3)). "Service-disabled veteran-owned small business concern"- (1) Means a small business concern- (i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and (ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran. (2) Service-disabled veteran means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16). "Small business concern" means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR Part 121 and size standards in this solicitation. The purpose of this clause is to ensure that the contractor and its subcontractors: (1) Are not biased because of their financial, contractual, organizational, or other interests which relate to the work under this contract, and (2) Do not obtain any unfair competitive advantage over other parties by virtue of their performance of this contract. (b) Scope. The restrictions described herein shall apply to performance or participation by the contractor, its parents, affiliates, divisions and subsidiaries, and successors in interest (hereinafter collectively referred to as "contractor") in the activities covered by this clause as a prime contractor, subcontractor, co-sponsor, joint venturer, consultant, or in any similar capacity. For the purpose of this clause, affiliation occurs when a business concern is controlled by or has the power to control another or when a third party has the power to control both. (c) Warrant and Disclosure. The warrant and disclosure requirements of this paragraph apply with full force to both the contractor and all subcontractors. The contractor warrants that, to the best of the contractor's knowledge and belief, there are no relevant facts or circumstances which would give rise to an organizational conflict of interest, as defined in FAR Subpart 9.5, and that the contractor has disclosed all relevant information regarding any actual or potential conflict. The contractor agrees it shall make an immediate and full disclosure, in writing, to the Contracting Officer of any potential or actual organizational conflict of interest or the existence of any facts that may cause a reasonably prudent person to question the contractor's impartiality because of the appearance or existence of bias or an unfair competitive advantage. Such disclosure shall include a description of the actions the contractor has taken or proposes to take in order to avoid, neutralize, or mitigate any resulting conflict of interest. (d) Remedies. The Contracting Officer may terminate this contract for convenience, in whole or in part, if the Contracting Officer deems such termination necessary to avoid, neutralize or mitigate an actual or apparent organizational conflict of interest. If the contractor fails to disclose facts pertaining to the existence of a potential or actual organizational conflict of interest or misrepresents relevant information to the Contracting Officer, the Government may terminate the contract for default, suspend or debar the contractor from Government contracting, or pursue such other remedies as may be permitted by law or this contract. (e) Subcontracts. The contractor shall include a clause substantially similar to this clause, including paragraphs (f) and (g), in any subcontract or consultant agreement at any tier expected to exceed the simplified acquisition threshold. The terms "contract," "contractor," and "Contracting Officer" shall be appropriately modified to preserve the Government's rights. (f) Prime Contractor Responsibilities. STATEMENT OF WORK Janitorial Service 1. The Contractor shall be responsible for satisfactorily managing and performing programs for cleaning as specified herein at the following location: National Weather Service Weather Forecast Office 7851 Dean Martin Drive Las Vegas, NV 89139 2. PRIMARY RESPONSIBILITY Todd Lericos, Meteorologist in Charge (MIC) - (702) 263-9744 x 222 (email: Todd.lericos@noaa.gov) - referred to as "office manager" throughout this document. Administrative Support Assistant (ASA) - (702) 263-9744 x221 The Contractor MUST meet with agency contact person prior to starting janitorial services. The purpose of the meeting is to provide an opportunity for the office manager to identify the office's unique security requirements (such as sign-in/out process), establish a working relationship, and address any lingering questions regarding expectations and requirements. The Contractor is responsible for conformance to the requirements established herein. Failure to conform to these requirements may result in the Government requiring the Contractor to perform the services again at no additional cost, or the price of the contract may be reduced to reflect the reduced value of the services performed. The Contractor is responsible for taking necessary action to ensure that future performance conforms to the contract requirements. 3. EQUIPMENT, MATERIALS, AND SUPPLIES The Contractor shall furnish and properly store in the areas designated by the office manager all equipment, materials, and supplies (MSDS for chemicals) necessary to perform the work, including, but not limited to, the following: Paper towels Paper toilet-seat covers and associated dispenser Toilet paper Hand soap Trash receptacle liners Cleaning supplies Cleaning equipment All products used are to be environmentally friendly, non-toxic, biodegradable, and made from renewable sources (not petroleum) to promote a healthy and green cleaning environment, when possible. NOTE: All paper products shall be made of recovered materials as per EPA guidelines at the following website: www.epa.gov/epawaste/conserve/tools/cpg/products/tissue.htm. A list of these products and their Material Safety Data Sheets (MSDS) will be supplied to the office manager for approval. The information will be kept on file in accordance with applicable guidelines of the OSHA Hazard Communications Standard. Requirements pertaining to recordkeeping will be strictly followed, with copy(s) provided to the office manager as necessary and upon request. The Government will furnish to the Contractor the utilities necessary to perform the work, as well as the keys needed to open the paper towel and other dispensers. 4. SECURITY BACKGROUND INVESTIGATIONS The Contractor is required to designate at least one (1), but no more than three (3), contractor personnel to work at the site. Prior to being permitted to work at the site, all Contractors entering the site must have first passed a fingerprint check, and have submitted a National Agency Check and Inquiries (NACI), also known as full background check, per the Commerce Acquisition Regulation (CAR) clause 1352.237-71 Security Processing Requirements - Low Risk (Apr 2010). A full copy of the clause is provided in the Clauses portion of the Purchase Order. The Contractor is responsible for providing, at its own expense, all information necessary to complete the security investigation, including official fingerprint charts. More information about the process to fulfill this security requirement is provided in Appendix A - Procedures for Requesting a Security Investigation. Personnel that do not receive a favorable result of either background check will not be permitted to continue to work at the site. 5. Conduct of Work The Contractor is responsible for satisfactorily managing and performing cleaning as specified. Services performed under this contract shall be performed during hours approved by the office manager. The Contractor will adhere to the sign-in/out process that is identified by the office manager during the mandatory pre-work meeting (see paragraph 2). The Contractor must maintain a secure environment while cleaning the facility. No one is allowed into the facility other than those individuals responsible for performing janitorial services and that have completed a security background investigation with favorable results. The Contractor is to provide all supervisory and general control over all day-to-day operations of his/her employees including control over all workers' duties, and these services must be satisfactory to the Government. The Contractor should bring any noted defect, disease or insect infestation of the premises to the attention of the office manager or designate. Services under this contract must be performed in such a way that there will be minimal interruption to or interference with the normal operation of Government business on the premises. The required result is to maintain the facility(s) in such a manner as to provide a clean, healthy and safe work environment for occupants of the premises. All rules concerning safety, smoking and the security of the building must be observed. The Contractor must keep track of completed duties i.e., daily, weekly, and monthly. 6. Square Footage/Site Conditions The area covered by this contract is a single-story office building of approximately 6,016 square feet. 7. Cleaning Schedule and Requirements The base year period of performance for this requirement is 12 months, beginning July 1, 2014 and ending June 30, 2015, with the option to extend up to four additional years. Cleaning will take place every Monday (cleaning to be performed before 8:00am), Wednesday and Friday (cleaning to be performed between the hours of 5:00pm - 5:00am). If a Federal holiday falls on one of these days, please refer to the section below, "Observance of Legal Holidays and Site Closure Information," for an alternate day of work. When the alternate day of work is Tuesday, work is to be performed before 8:00am. Observance of Legal Holidays and Site Closure Information The Contractor hereby agrees to observe the following Federal holidays for the period of July 1, 2014 - June 30, 2015, plus any other day off work designated by Federal Statute, by Executive Order, or by Presidential proclamation: New Year's Day Labor Day Martin Luther King's Birthday Columbus Day President's Day Veteran's Day Memorial Day Thanksgiving Day Independence Day Christmas Day When any holiday falls on a Saturday, the preceding Friday is observed; when any holiday falls on a Sunday, the following Monday is observed. All personnel assigned to this contract shall limit their observation of holidays to those set forth above. In each instance, the Contractor agrees to continue to provide sufficient personnel to perform requirements of any critical tasks already in operation or scheduled, and shall be guided by the instructions issued by the Contracting Officer or the office manager. In the event of a site dismissal or closure due to weather, emergency or other circumstances, the Contractor is instructed to contact the office manager or designee for guidance. 8. Requirements Special note: the raised tile floor in the operations and equipment rooms has a special, static-free lamination to protect the equipment in the area. All sweeping of this floor must be by the dustless pickup method using either the laundered or the disposable type of treated cloths, as appropriate. All dusting must be done by the damp or treated cloth method. The Operations areas have many computer cords that extend from the floor. Cleaning must be done within 12" of these cords. No tables, equipment, or cords shall be moved to clean around or behind them. All methods and materials used in all cleaning work must leave surfaces free of debris, film, streaks, spots, and stains. The Contractor must dispose of all waste in the proper recycle or trash receptacles out of the building. a. Work to be performed three times a week on Monday, Wednesday, and Friday: i. All trash receptacles must be emptied and returned to their initial location. Empty and clean ash receptacles located outside of the two building entrances. Remove all trash to appropriate outside receptacles (Recycle or Trash receptacle). Wash or damp wipe inside and outside of all trash receptacles presenting a soiled or odorous condition, leaving the receptacle free of soil and odor. Replace liners when torn or soiled with a fresh liner. ii. Thoroughly vacuum all carpeted areas and rugs, leaving them free of dust balls, dirt, and other debris. Floor edges will also be left free of debris. Move and vacuum under all easily movable objects (chairs, trash receptacles, tables on wheels, typing stands, boxes). Put all items that were moved back in place. Vacuum cleaner contents will be removed to a dumpster. Carpet spot cleaning is required on an as-needed basis for spots equal to or larger than the size of a dime. iii. Wet-mop all uncarpeted floors, including hallways and the break room, using a detergent solution. Remove all oil, tar, and stubborn dirt. Cords, tables, and equipment in the operations area are not to be moved to clean behind. iv. Thoroughly dust-mop all non-carpet areas. Move a dust mop under all easily movable objects (chairs, waste receptacles, tables on wheels, typing stands, boxes). Put all items that were moved back in place. Dust mops must be treated with water based dust control chemicals. The Contractor will contain the dust and dirt and remove it to a dumpster. v. In the restrooms, sweep and damp mop floors with a germicidal solution, paying special attention around sinks, toilets, and urinals. Floors will be left absent of debris. Clean and disinfect fixtures, handicap rails, sinks and walls around sinks, countertops, mirrors, toilets, urinals, and walls around urinals. Empty and disinfect all sanitary napkin receptacles. Refill soap, towels, toilet paper, and seat cover dispensers. Maintain a week's supply of soap, towels, toilet paper and seat protectors in the janitorial closet. vi. In the break room, clean and disinfect fixtures, sinks, walls around the sink, countertops, and table. vii. Clean and disinfect all drinking fountains including the drains and exterior surfaces of fountain. Drinking fountains must be free of streaks, stains, spots, smudges, scale, and other obvious soil. viii. Clean and polish glass entrance doors (2) inside and out. b. Work to be performed weekly, on Wednesday: i. In the break room, wipe exterior of all appliances including recycle bins, trash receptacles, and spot-clean walls. c. Work to be performed monthly, on the first Friday of the month: i. Damp-wipe all office fixtures, window sills, walls, doors, splash guards, baseboards, and ledges. Damp mop plastic chair mats. Leave window blinds free of dust. Remove cobwebs. ii. Dust or polish high and low surfaces, ledges, cabinet tops, and ventilators on which dust gathers. Clean and polish ¬all¬ finished wood surfaces in the building, including but not limited to, the table and book cases in the conference room. iii. Clean all interior windows and other existing interior glass using a commercial glass cleaner. iv. In the restrooms: clean exterior of pipes beneath all sinks; polish chrome; clean light switches, doors, and kick plates; spot clean walls. Invoicing The Contractor will provide a "proper" invoice on a monthly basis, in arrears, in accordance with the Prompt Payment Act. Improper invoices will be returned and payment will not be made until receipt of a proper invoice.
